*The Master is in overall command of all personnel and


operations aboard the ship. ... He has the authority and
responsibility to make decisions with respect to safety
and pollution prevention and to request assistance from
the Ship Operations Group or any other appropriate
source.

*A Document of Compliance ( DOC ) is a safety certificate


issued to the shipping companies; in compliance with the
ISM Code 1998 under chapter IX of SOLAS. ... The main
objective of such audits is to verify company compliance;
to provide for safe practices in ship operation and a safe
working environment.

*Entered into force on 1st July 1988 as SOLAS Chap. IX,


the ISM provides an international standard for the safe
management and operation of ships and for pollution
prevention. Its objectives are ensuring safety at sea,
preventing human injury and fatalities and avoiding
damage to the environment.
It is applicable to all oceangoing ships over 500 GT and to
the owner or management company that has assumed
the ship operation responsibility.

Service Details

*The ISM Code requires owners and operators to set in


place a Safety Management System (SMS) embracing its
objectives and involving the totality of the Company’s
operations and managed ships.
The SMS allows a Company to measure its performance
against a documented system, and it enables a Company
to identify areas of improvement in safety practices and
pollution prevention measures.

*The ISM certification procedure consists of the


following steps.
For the Company: Document of Compliance (DOC):
•review of Safety Management System documentation
•interim audit for issuance of interim DOC (valid one
year)
•initial audit and issuance of full term DOC (valid five
years)
•after the full term certificate issuance, 4 annual audits
and DOC renewal audit after 5 years.
For the Ship: Safety Management Certificate (SMC):
•after DOC issuance to the company, Interim audit for
issuance of interim SMC, valid six months
•initial audit and issuance of full term SMC (valid five
years)
•intermediate audit follows 3rd year of the certification
period
•SMC renewal after 5 years.

*The main objective of the SOLAS Convention is to


specify minimum standards for the construction,
equipment and operation of ships, compatible with their
safety.

The MARPOL Convention is the main international


convention covering prevention of pollution of the
marine environment by ships from operational or
accidental causes. It is a combination of two treaties
adopted in 1973 and 1978 respectively, and updated by
amendments through the years.

*Purpose STCW certification was created to promote
safety of life and property at sea and to protect the
marine environment. It establishes internationally
accepted standards of training and certification of
seafarers, ensuring that crew are qualified and fit for
duties at sea.

*The permit-to-work is a documented procedure that


authorises certain people to carry out specific work
within a specified time frame. ... It sets out the
precautions required to complete the work safely, based
on a risk assessment.

*There are seven main types of work permits: Hot Work


Permits, Cold Work Permits, Height Work Permits,
Confined Spaces Work Permits, Excavation Permits, and
Chemical Work Permits. Each work permit is categorized
depending on the nature of the job and the hazard
involved in it.

*An “Accident” is defined as an unplanned event that


results in personal injury or property damage. ... A near-
miss is usually caused by a series of dangerous conditions
that when unnoticed will eventually result in an accident.
*The Dynamic positioning - DP automatically maintain
the vessel's position and heading using its propellers and
thrusters. The vessel's response to these forces, i.e. its
changes in position, heading and speed, is measured by
the position-reference systems, the gyrocompass and the
vertical reference sensors.

*The three most common rental insurance policies are


the DP1, DP2, and DP3. The DP1 is the most basic
landlord insurance policy, providing very bare bones
coverage. The DP3 is the most extensive landlord
insurance policy, providing the broadest and deepest
coverage.
*Operational activity planning applies to all sectors of
offshore marine operations, including offshore drilling,
project & construction vessels and offshore supply
vessels – especially to dynamically positioned (DP)
vessels.
DP & Marine Assurance Norway AS provide Critical
Activity Mode (CAMO), Task Appropriate Mode (TAM)
and Activity Specific Operating Guidelines (ASOG).

In accordance with IMCA recommendations, the


guidance is presented to the Dynamic Positioning
Operator (DPO) as a quick reference tabular format.
These abbreviated documents provide a cross reference
to the DPO and other relevant engineering and bridge
personnel.

Critical Activity Mode (CAMO)

Critical-Activity-Mode

The Critical Activity Mode of Operation (CAMO) shows


how to configure the vessel’s DP system, including power
generation & distribution, propulsion and position
reference systems.

Task Appropriate Mode (TAM)


Task-Appropriate-Mode

Task Appropriate Mode is the configuration that the


vessel’s DP system may be set up and operated in,
accepting that a single failure could result in exceeding
the worst case failure and could result in blackout or loss
of position.

*Failure Modes and Effects Analysis (FMEA) is a


systematic, proactive method for evaluating a process to
identify where and how it might fail and to assess the
relative impact of different failures.

Activity Specific Operating Guidelines (ASOG)

Activity-Specific-Operating-Guidelines

Activity Specific Operating Guidelines (ASOG) are


generally presented in tabulated format and set out the
operational, environmental and equipment performance
limits necessary for safe DP operations while carrying out
a specific activity.
