I. Preliminaries
1. The meeting begun at 3:00 P.M. Mrs.PAZ G. TOPIA, the Grade 6 teacher of Suba ES formally lead
the prayer and at the same time hosted the conference.
2. The second part is the Roll Call guided by the grade 1 teacher of Suba ES Mrs. Jogie P.
Marcellana.
100% of the teachers are present.
3. The School Principal acknowledged the presence of the teachers.

II. MEETING PROPER


1. The school principal presented the statement of purpose of that particular meeting by
giving the introduction and the statement of purpose of the project “2R and I”. Repainting,
restoring, and Improving the school wall.

2. The school principal requested the school Physical Coordinator, Mrs. Sarah B. Dela Cerna
to prepare the Program of Work of the said project.

3. Discussion of the Implementation by the School Principal. Project Proponent will be Mrs.
Cheryl E. Roda since it is near to her classroom, she can do the supervision. A PTA
meeting will be conducted on August 08, 2022 at 3:00PM at the conference room.

4. Acknowledgement was given by the school guidance coordinator, Mrs. Yoji F. Luague.

III. The meeting was adjourned at 5:00PM.

I. Preliminaries
1. The meeting begun at 3:00 P.M. Mrs. Analyn Rocamora, the PTA President of Tangbo ES
formally led the prayer and at the same time hosted the conference.
2. The second part is the Presentation of Parents by grade level guided by the teacher
representative of Tangbo ES Mrs. Anna Liza C. Sesaldo.
3. The School Head acknowledged the presence of the parents.
II. MEETING PROPER
2. The school head presented the statement of purpose of that particular conference
and presented the E-SIP crafted by the SPT Team and was already submitted to
the division office for approval.
3. The school head also reads the memo on crafting the E-SIP.
3. Discussion of the Mission, Vision, and Core Values of the School.
 PTA President
The Faculty president Mrs. Sandra Bilocura shared her own vision of the school
being the representative of the teachers. The PTA President also shared her ideas
wherein pupils must have complete uniform and that facilities should be more
accessible to pupils.
4. Discussion of School PIA’s
 School Perimeter Fence – 50% is still complete
 Undernourished pupils- high percentage of undernourished pupils
 Reading literacy level of the pupils- 29% are in frustration level
5. Suggestions on how to solve the PIA’s
The school head asked ideas and opinions from the parents on how the school
with the parents and other stakeholders could solve this PIA’s of the school. It is
agreed that parents will support in the abolishment of the three (3) PIA’s of the
school which was specified to be given importance within 3 years.

III. The meeting was adjourned at 5:00PM.

I. Preliminaries
The meeting begun at 3:00 P.M. It was formally opened with an invocation lead by Mrs. Anna
Liza Sesaldo which was followed by the singing of the National Anthem.

II. AGENDA
1. Meeting with the new School head and the parents. Greetings from each other.
2. Election and oath of the new set of officers for the SY 2019-2020.

3. School Gardening.
School Gardening will be monitored by the school head and that advisers must give time for the
students to cultivate and to plant vegetables in their garden. In connection, parents were also
asked to support the GPP of the school to supplement the feeding program of the school.
4. Repair/rehab of School Fence.
The school fence was already started by the former principal so the GPTA will be having their
own meeting for the continuation of the school perimeter fence as their support.

The principal presented the School Years’ Calendar of Activities for the SY
2019-2020.

2. Pupil’s Handbook
The pupil’s handbook was presented and explained to the parents by the
principal.

3. Feeding Program
The principal asked support from the parents to help for the cooking and
feeding of those pupils who are undernourished.

4. Tree Planting
The parent’s agreed on the schedule of tree planting as presented to them by
the NGP Coordinator Mr. Sandra Bilocura.

The Agenda was presented by our District Supervisor Mr. Victoriano I. Borla.
1. Supervisor’s School Visit
 The district supervisor relayed to the school heads his visit to San Sebastian ES a day
before the meeting.
 There will be a school/ district evaluation checking of forms. In connection, school heads
are required to submit list of school checkers/ evaluators to the district office. For the
Grades 1-6 ORV 3rd quarter will be the basis for the reading evaluation while for kinder
MTB pantig or word and phrases.

2. School Building Assessment by GSIS


 Forms to be accomplished by each schools has been distributed by the supervisor and
then informed everyone that all entries must be encoded.

3. Saturday classes must be monitored by the school heads. Permit was already approved.
 No. of School days for the SY 2018-2019 by Ma’am Austrid, the District Guidance Coor.

ELEMENTARY SCHOOL, DISTRICT OFFICE ON February 20, 2019.

I. Preliminaries
1. The meeting begun at 9:00 A.M. Mrs. Crispina B. Tatang, the TIC of Basak ES formally opened it
with a prayer.
2. The second part is the Roll Call guided by the principal of Tangbo ES Mr. Paquito S. Itao.

II. AGENDA
The Agenda was briefly presented by our District Supervisor Mr. Victoriano I. Borla.
1. L & D, CPD, and L&D Coordinators
 The district supervisor collected the list of school coordinators from the school heads.
 Mr. Borla also required the school heads to submit two copies of designation order in the
district office.

2. Summer INSET Budget Proposal


 It was presented and discussed by Dr. Analyn Inoferio, the district HRD.
 Informed the school heads that the result of BEA has been posted on FB which was
illegal and now has been under investigation.
 TIP was scheduled on May 16-18, 2019.
 INSET was scheduled on May 27-31, 2019.
 TOT will be on May 20-24, 2019.
 The different topics for the INSET and TIP speaker has been presented, re-arranged,
and was approved by everybody.
 Demonstration during Summer INSET was equally distributed to schools with T3
teachers. There are 5 clusters so there are also 5 demonstrators. 3 from the Elementary
while 2 from the Secondary.
 The following Schools will represent one demonstrator on the Summer INSET:
Elementary - Basak ES, San Sebastian ES, Kamburoy ES
Secondary – Samboan National HS and San Sebastian Nationa HS
 Process Observers per cluster as well as the members of the training management team
has been arranged and agreed by everybody.
 Materials for the training will be charged from school MOOE.
 Every school are required to give 5 minimum dark green expanded envelope and 10
long folders for the said training.
 The 5 Clustered Schools for the Summer INSET 2019
Cluster 1 – Samboan National HS, Dalahikan ES and Suba ES
Cluster 2- San Sebastian National HS, Bulangsuran IS HS, PBUC National HS
Cluster 3 – Bask ES, Cambigong Es, Monteverde, Tangbo ES, MLQ ES, Kamburoy ES
Cluster 4 – Kalatagan ES, Jumangpas ES, Bulangsuran IS ES
Cluster 5 – Samboan Central ES and San Sebastian ES
 TIP will only be for the newlyhired teachers from June 2018 to April 2019. Those who
were not able to join the TIP last school year were tasked to submit their explanation
letter to the division office so that they can acquire the permit in attending the TIP.

The district supervisor declared lunch break at 12:00noon.

At 1:00PM everyone prepared the documents of INSET and TIP for Division submission.

I. Preliminaries
1. The meeting begun at 1:00 P.M. Mrs.Dominiciana F. Hisoler, a school head of Jumangpas ES
formally opened it with an Invocation.
2. The second part is the Roll Call guided by the principal of Dalahikan ES Mrs. Oliva G. Balbona.

II. AGENDA
The Agenda was discussed by our District Supervisor Mr. Victoriano I. Borla.
Cebu Provincial Meet 2018
 Kick off date will be on January 5, 2019. Meeting time will be at 7:00AM. Departure time
will be at 8:00AM. Kick Off venue will be at Central School. Participants will bring their
own lunch and snacks. For dinner, rice will be provided.
 Bldg. Quarter will be at Buluang ES, Compostella, Cebu.
 For the elementary, 85 persons were included in the budget provided by the municipality
while more than 104 participants for the secondary.
 P30,000.00 for the bus will be shouldered by Ma’am Jenny H. Angulo, the school head
of Cambigong Elementary School provided that refund will be given immediately as soon
as the budget is available. For the secondary, the amount has been divided by the
secondary schoolheads.
 Kitchen utensils will be provided by the kitchen in-charge headed by Mr. Danilo
Limpahan.
 At the moment, only P30,000.00 cash available for the elementary and secondary
uniform. Checks for the said event don’t have mayor’s signature yet.
 School heads appearance will be provided by the district while coaches will be coming
from the Division office. If someone will go home before Friday January 11, 2019, they
must have to ask permission from Sr Borla for their appearance.
 The ff. persons are the kitchen in charge.

II. AGENDA
The Agenda was discussed by our District Supervisor Mr. Victoriano I. Borla.
1. Division Memo 848
There will be a Master Teacher’s Summit on December 17 to 19, 2018. Venue will be at
DepEd Ecotech Center.
2. IPCRF REVIEW
Period of Review will be June to March 2019. School heads period of review will be the
same with teachers. No more Fiscal Year. 10 KRA’s for school heads while 5 for teachers.
3. NCAE
Examination for Grade 9. Schedule will be on December 12 to 13, 2019.
4. Service Credits
Allowable usage for the teachers is only 15 days excluding service credits from Election.
Can be offset only for Sick Leave.
5. ALS
ALS Teachers should have a weekly lesson log. PSDS and school heads should do the
monitoring and communicate with ALS teachers regarding ALS activities.
6. Avoid text messages to our Superintendent Dr. Rhea Mar A. Angtud. School and District
Grievance Committee must be functional. They were tasked to monitor also the School MOOE
utilization.
7. School heads are not allowed to do the canvassing and purchasing.
8. CID Meeting of PSDS
All PSDS’s are required to bring Accomplishment report and CIGPs during the said
meeting.
9. Financial Literacy
Slide is available at CID office.
10. Provincial Meet for SY 2018-2019
As agreed by everyone Dr. Loreta Taan and Mrs. Jenny Angulo will be the officers to be
entrusted with receipt, care, and disbursement of funds while schoolheads are the purchasers.
All male teaching and non-teaching will be in charge of the kitchen and dishwashing. More
clarifications regarding Provincial meet will be discussed on the next meeting.

The Agenda was discussed by our District Supervisor Mr. Victoriano I. Borla.
1. DSMEA 2nd Quarter
DSMEA 2nd quarter will be on November 23, 2018. Venue as agreed by everybody will be at San
Sebastian Elementary School. SMEA Coordinator and School head are requested to attend.
2. MOOE and other checks from the Division Office
As of the moment the supervisor is unable to claim any checks in the disbursement office due to
stale checks of some schools. In connection, the supervisor received Show Cause Order and
required to submit his explanation letter on the matter to the Division Superintendent.
3. WFP for Feeding
WFP of schools with OPEX for the wasted and severely wasted that have been approved by the
accounting office was misplaced and cannot be found. The school heads are requested to
submit a new one to Mr. Bethany Uraca the district ADAS as soon as possible.

1. Preparedness of the school for the Opening of Classes. Brigada Eskwela Implementation
Plan must be submitted.
2. Best Practices report for the High School are required.
3. EVALUATION SCHEDULE on April 30 to May 4, 2018.

4. Food for the Evaluation on May 8, 2018

Fish – 1,000 with spices
Rice – 5kls. – P250.00
Fruits – 500.00
Total of 8,550.00 divided by 19 schoolheads is P450.00 plus P50.00 for the give aways. It
was agreed to donate P500.00 per schoolhead. Deadline will be April salary.

5. OTHER MATTER
The SPSTA Treasurer is a teacher of Bask ES so Mr. Oscar E. Noel the schoolhead of the said
school, asked the schoolheads for clarification regarding the retirement gift from the SPSTA
since some retired teachers did not pay the said contribution.
It was agreed that those retired teachers who did not pay for the aid can’not also avail the
retirement gift and hospitalization aid.

1. Trainings of Teachers in Filipino must be reported.


Without training specially for those schools with aged teachers in the service is not accepted.
Template must be submitted to be consolidated on July 20, 2018.
2. PESS
a. School Intramurals will be on July 20-21. It will be a simultaneous activity.
b. District Meet will be on August 24 to 25, 2018.
Games and Activities:
Volleyball – boys and girls
Basketball – boys
Chess – boys and girls
Badminton – boys
Running same as last year
Miss Sportsfest then Vocal solo boys and girls OPM will be the intermission number during the
activity.
January 1, 2006 cut off in Elementary
3. Biometrics Time- In and out depends on the agreement of the school’s staff.
DTR must be aligned with the logbook and biometric print out. Teachers should tog the time
everyday on the DTR and logbook and affix the signature.
Submission of the monthly DTR and monthly report will be every 1 st and 2nd day of the month.
Third day will be considered late and it is further agreed that the in charge of that DTR will be the
one to submit it to the DO. 5th is the last day of submission to the DO.
4. RPMS
Required the ff.:
a. IPCR. Time frame will be June 2018 to May 2019.

b. Required to make CISP (Curriculum Implementation Supervisory Plan). Time frame will be

June 2018 to May 2019. Deadline will be on the 1st day of August 2018.

c. ISP

In observing write the Grade level and the subject. Observation should be one teacher in the

morning and one in the afternoon. Choose the best performance of the teacher in every quarter

to be attached on their RPMS.

d. Monthly Work Plan and Accomplishment


Included the Target, Actual Target, Accomplishment and Remarks every week. Must be
approved by the DO together with the ISP.

5. GSIS
GSIS ID will be claim at the DO by the district supervisor only for those who are still using the
Unionbank. List of Teachers Loan Status are provided in the district and advised to inquire it in
the GSIS office.

6. Other Matters
1. 4th Executive Conference Hosting on August 02, 2018
Emcee will be Mrs. Bernardita Arias the school head of Samboan National High School.
Agreed to propose to Ginatilan district that we will be the in charge on Emceeing, Program, ICT
Operator, Phil. National Anthem, Registration and Attendance, and the Holy Mass while for the
Ginatilan district they will be in charge of Decoration, Folder, After Care (Before and After) and
Minutes Making.
2. Report on Science based from Memo 444, S. 2018
Report will be complied by all School Science teachers. Deadline on Monday afternoon July 16,
2018.
3. Quadrants of teachers Performance
Teachers’ performance will be classified according to quadrants based on the latest RPMS
rating.
4. SUKOD Briefing on July 18, 2018.
All school heads are required to attend.
5. Teachers Night Presentation
Social dances was agreed by everybody.

Agreed to hire a stage decorator and agreed further to pay ₱5,000.00.

Stipend for the priest is ₱1,500.00 and for the accompaniment is ₱500.00.

As being relayed by Mrs. Cabuenas the schoolhead of Bulangsuran IS, according from Mam
Angtud we are only allowed to hire outside referee providing that there should be sufficient
documents of the said outside referees. In connection with this, it is being agreed by everybody
that there will be no more 100pesos contribution from the school for the referees instead
officiating officials will be the teachers from elementary for the high school while highschool
teachers will officiate the elementary game. On the other hand, event coordinators will finalize
the scheduling of the game. Moreover, referees decision is final.
3. Other Matters
Souvenir Program
Big Schools must take the one whole Souvenir worth P3,000.00 while Schools with less than 5
teachers will take the Quarter Page.

District Staff Treasurer Mrs. Jenny Angulo, the school head of Cambigong ES.

The Committee for the night was reflected on the program. The same committee as last year.

2. Expenses for the Night


According to the in charge of the budget, the P25, 000.00 that will be provided by the
municipality will only be available on Thursday September 27, 2018. Therefore, in connection
with the delay of the budget, the school heads agreed to lend any available amount from their
own pocket that will be returned immediately as soon as the money from the municipality is
available.
