Portfolio Shell Creation

I am an experienced registered nurse with over two decades of healthcare experience in

acute care, long-term care, and hospice settings. I am passionate about providing exceptional,

patient-focused care to a diverse population. My current education in the BSN program at

Western Governors University has allowed me to further develop my knowledge and skills in

these areas. I am currently working towards graduating from the program and I am eager to

apply my knowledge and skills to my nursing practice. My portfolio includes a breakdown of my

skills and experience and a brief overview of four key areas: quality and safety, evidence-based

practice, applied leadership, and community and population health. My portfolio will

demonstrate my commitment to quality patient care and my ability to provide safe, evidence-

based, and compassionate nursing care. I am also looking forward to continuing to develop my

skills and knowledge and to sharing my professional experience with my peers, colleagues, and

future employers.

Quality and Safety

Quality care and safety of patients has been my focus in my nursing practice. I have

worked in acute, long-term, and hospice care settings. I understand the importance of delivering

patient-focused care to a diverse population and the need to be knowledgeable about quality and

safety standards. My experience in the healthcare setting has equipped me with the skills and

knowledge to ensure that quality and safe care is provided to my patients. Therefore, with my

vast skill and knowledge, quality care and safety is guaranteed.

I am familiar with the use of Electronic Medical Records (EMR) and understand the

importance of documenting accurately and timely. I possess vast knowledge on medical

administration and strictly follow physician’s instructions. I understand the importance of

assessing patients and reporting changes in care and outcomes to the physician. I acknowledge

the significance of providing emotional support to the patient and their families while at the same

time designing duties to the relevant healthcare staff. Therefore, with accurate documentation of

medical records. qua;ity care and safety of patients is guaranteed.

I know the importance of obtaining verbal certifications from physicians and obtaining

electronic consent. I understand the importance of providing end-of-life care to terminally ill

patients and educating them and their families on end-of-life issues such as symptom

management, pain control, and DNR. I am also familiar with the need to provide restorative

therapies such as feeding, ambulation, ROM, and ADLs and assist patients with using adaptive

equipment. Overally, my healthcare experience has given me the knowledge and skills to ensure

quality and safe care is provided to my patients. I understand the importance of following

standards and guidelines and am committed to providing the best care possible thus providing

quality and safety care to the patients.

Evidence-Based Practice

Evidence Based Practice constitutes one of the most critical components of nursing. It is

a process of integrating evidence from research and clinical experience to inform decisions about

patient care (Lam & Schubert, 2019). As a nurse, I have been committed to using EBP in my

practice. My two decades of healthcare experience have enabled me to become well-versed in

using EBP. I have worked in acute care, long-term care, and hospice care, which has provided

me with a wide range of experiences. As a registered nurse at AccentCare Hospice, I provide

assessments when individuals are terminally ill to work closely with physicians to determine

whether a patient is eligible for hospice care. Similarly, I also educate patients and families on

end-of-life issues, including symptom management, pain control, and DNR. I am also skilled in

obtaining verbal certifications from physicians, including obtaining electronic consent.

Therefore, the incorporation of EBP in my nursing practice has significantly improved the well

being of the patients and their families.

During my nursing practice at South Florida Baptist Hospital, I executed various

responsibilities. The roles included administering medications as prescribed by the physician,

monitoring labs and calling critical labs to physicians, assessing patients and documenting

changes, providing care outlined in the patient's plan of care, providing emotional support to

patients and families, calling physicians to obtain orders, delegating tasks to appropriate staff,

calling consults to physicians, drawing labs, and administering PO/IV medications as prescribed

and according to schedule. Further, at Blake Medical Hospital, I was responsible for

administering medications as prescribed by the physician, monitoring labs and calling critical

labs to physicians, assessing patients and documenting changes, providing care outlined in the

patient's plan of care, providing emotional support to patients and families, calling physicians to

obtain orders, delegating tasks to appropriate staff, calling consults to physicians, drawing labs,

and administering PO/IV medications as prescribed and according to schedule. Therefore, I have

executed various roles in two hospitals that have significantly improved the well being of the

patients and the medical fraternity as a whole.

At Mentor Lifepath Hospice, I provided supportive care to hospice patients living in LTC

facilities that are living with end-of-life illnesses. This included assisting with hygiene care,

providing emotional support, EMR, assisting the team secretary with scheduling HHA on the

team, transferring/lifting patients out of bed, notifying the nurse when there were significant

changes in patient condition, and collaborating with team members in IDT. At University Village

Nursing Inn, I assisted with restorative therapies and ADLs, assisted patients in transferring out

of bed/chair gait belts, and assisted patients in using adaptive equipment. The use of EBP is

essential in providing the best care possible to patients. I am on the forefront on staying updated

with the current guidelines on evidence based practices. I am committed to providing the best

care possible to my patients and always strive to improve my practice through EBP.

Applied Leadership

My experience in Applied Leadership has been incredibly rewarding. I have had the

opportunity to use my skills to help others while also learning how to be a better leader. Through

my experience in healthcare, I have learned how to communicate effectively with patients and

their families while also being able to delegate tasks to other healthcare professionals. I have also

learned how to develop and maintain positive relationships with my peers and colleagues, which

is essential for any successful team. Therefore, applied leadership has significantly contributed to

my success in relating well with people in the healthcare fraternity.

One of the most important aspects of Applied Leadership is the ability to make decisions

(Flavian et al., 2019). I have learned how to assess a situation and make informed decisions

based on the patient's best interests. I have also learned how to prioritize tasks and delegate

appropriately, which is essential for providing quality patient care. Additionally, I have learned

how to effectively collaborate with other healthcare professionals and work together to develop a

patient care plan. I have also understood the importance of cultural competency and how to

communicate effectively with individuals of different cultural backgrounds. I have learned how

to be sensitive to the needs of individuals from different cultural backgrounds and how to

provide culturally competent care. Additionally, I have learned how to be flexible and adjust my

approach to meet the patient's needs.

Overall, my experience in Applied Leadership has been incredibly rewarding. Through

my experience in healthcare, I have gained valuable skills that will help me to be an effective

leader in any setting. My experience has prepared me to be a successful leader in any healthcare

setting.

Community and Population Health

My healthcare experiences have given me a deep understanding of the importance of

community and population health. As a registered nurse in acute care, long-term care, and

hospice care settings, I have witnessed firsthand how different populations of people are affected

by their health. I have worked with individuals from a variety of cultural, ethnic, and religious

backgrounds and have been able to gain a greater understanding of how each population's health

needs differ. I have also seen how the quality of healthcare in certain communities can

significantly impact the health outcomes of the population.

For example, while working as a hospice aide, I provided supportive care to individuals

living in long-term care facilities. I witnessed firsthand how the quality of care, or lack thereof,

can directly impact a population's health outcomes. I provided emotional support to patients and

families, educated them on end-of-life issues, and provided symptom management and pain

control. My experience in these settings has shown me the value of providing quality care to

ensure the health of an entire population.

I have also gained an understanding of the importance of population health through my

work in restorative care. As a restorative aide, I provided care to individuals with various

disabilities. I assisted with restorative therapies, assisted with activities of daily living, and

provided emotional support. This experience has shown me the importance of providing quality

care to ensure the health of a population. Overall, my healthcare experiences have given me a

deep understanding of the importance of community and population health. I emphasize on the

well being of the population and the community by providing quality care to the patients.

Conclusion

The portfolio showcases my dedication to excellence and safety, the use of evidence-

based practices, leadership in action, and a focus on both individual and public health. I have

developed a strong foundation of knowledge and skills through my clinical experiences,

educational background, and professional practice. I possess the abilities essential for delivering

excellent nursing care to those under my care. I am committed to providing the highest level of

care and am dedicated to continuing my professional development. I am eager to apply my skills

and knowledge to improve my patient's health and quality of life. I have the necessary skills and

knowledge to be an effective nurse leader and am excited to continue to grow and develop my

practice.
