    CONTRACT OF LEASE

    This CONTRACT OF LEASE made and entered by and between the Local Government of
    Loon, Bohol with official address of Cogon Norte, Loon, Bohol represented by its Municipal
    Mayor ELVI PETER L. RELAMPAGOS, herein referred to as the LESSOR.

    -AND-

    _________________________________, ______________ of the deceased named


    _________________ who died on _________________________ at ____________________,
    Bohol, hereby referred to as the LESSEE.

    -WITNESSETH-

    WHEREAS, the LESSOR is the owner of the Municipality Cemetery, located at Cogon Sur,
    Loon, Bohol.

    WHEREAS, the LESSEE is desirous to lease the Lot within the Municipality Cemetery from the
    LESSOR, to be used as Burial Ground , for her/his Husband/Wife, and is willing and able to
    lease the same from the LESSOR.

    NOW THEREFORE, for and in consideration of the foregoing premises, the parties hereby agree
    as follows;

    A). Lease of Niche for Cadaver:

    1. This contract shall cover for a period of Ten (10) years from
    _______________________ and end on __________________ with a fee of Five Thousand
    Pesos (P 5,000.00) as monetary consideration for the niche construction and execution of this
    contract and renewable for Five (5) years upon payment of Two Thousand Five Hundred Pesos
    (P2,500.00). This shall be collected by the Municipal Treasurers Office.

    2. Arrays of niche shall be prepared and constructed by the Municipal Engineering


    Office.

    3. The Office of the Municipal Engineer is tasked to assign specific niches to be occupied
    by the LESSEE in scheduled sequence;

    4.Upon expiration of this contract, the skeletal remains shall be transferred to the niches
    for skeletal remains and subject to separate guidelines and fees.

    5. The LESSEE shall be responsible for the observance of cleanliness of the premises.

    B). Lease of Niche for Skeletal Remains:

    1. This contract shall cover for a period of Five (5) years from
    _______________________ and end on _____________________ with a fee of Four Thousand
    Pesos (P 4,000.00) as monetary consideration for the usage of niche for skeletal remains
    comprising of a maximum of Four (4) remains and renewable for Five (5) years upon payment of
    Four Thousand Pesos (P4,000.00). This shall be collected by the Municipal Treasurers Office.

    2. Failure to pay the renewal fee shall automatically cause the transfer of the remains to
    the common skeletal grave.

    3. Arrays of niche shall be prepared and constructed by the Municipal Engineering


    Office.

    4.Upon expiration of this contract, failure of the LESSEE to initiate the transfer of the
    remains to the niches for skeletal remains shall automatically cause the LGU to transfer the
    remains to the common skeletal grave.

    5. The LESSEE shall be responsible for the observance of cleanliness of the premises.

    C).Lease for Ground Burial Lot:

    1. This contract shall cover for a period of Ten (10) years from
    _______________________ and end on __________________ with a fee of Three Thousand
    Pesos (P 3,000.00) per square meter and renewable for Five (5) years upon payment of One
    Thousand Five Hundred (P1,500.00) per square meter. This shall be collected by the Municipal
    Treasurers Office.

    2. The LESSEE incurred a total of ______ square meters of lot subject to the payment of
    ___________________________.

    3.Upon expiration of this contract and failure to execute a renewal of contract, this shall
    cause the transfer of the skeletal remains to the niches for skeletal remains, subject to separate
    guidelines and fees or at the common skeletal remains grave.

    3. The LESSEE shall be responsible for the observance of cleanliness of the premises.

    That the parties hereto shall bind themselves to honor and respect all the terms and conditions set
    forth in this CONTRACT of LEASE which they have entered into voluntarily and of their own
    free will.
