ACCOMODATION BUILDING MOST OF WORKS ARE NOW GOING ON FOURTH FLOOR. THE PROJECT MANAGER OF THE
COMPANY IS VISITING THE SITE MONTHLY AND HE MAINLY FOCUS ON PROGRESS AND NOT FOCUS ON SAFETY.
ONE DAY SCAFFOLDING PIPE FALL ON WORKER WORKING IN GROUND FLOOR AND SUSUTAINED SEVERE INJURY.HE WENT
THERE FOR INVESTIGATION AND STARTED ACCIDENT INVESTIGATION.BY TALKING WITH THE WORKERS ONE OF THE WORKER
LIKE THAT NEAR MISS HAPPENED BEFORE NO ONE IS REPORTING AND NO INVESTIGATION DONE ON IT, REPORTING
PROCEDURES ALSO COMPLICATED EXCESSIVE PAPERWORK.
INTERNAL AUDITS ARE DONE BY THE PROJECT MANGER BUT IT IS NOT EFFECTIVE.
THE MANAGEMENT HEALTH AND SAFETY PERFORMANCE REVIEWS DO NOT CONSIDERED WIDE RANGE OF ISSUES FOR IT
WORK EFFECTIVELY.

Conduct perioding for all employees including physical and mental health
Establish security procedures and guidelines for all employees including access controls security cameras and security

Train employees on incident response procedures and how to report suspicious activities

By investigating the near miss we could have predicted the upcoming hazards and the possible other potential hazards

3. WHAT ARE BARRIERS OF REPORTING NEAR MISS WHERE APPILCABLE GIVE ANSWERS FROM SCENARIO
The belief of the project manager that the near miss was not that serious
The project manager neglected the danger of the near miss in order to avoid the complicated paperworks
Probably the belief of reporting a near miss may result in the reputation of the company
Reporting a near miss incident can be time consuming and resource intensive especially if the incident requires detailed
analysis and coordination with the multiple stakeholders

internal auditors are often employees of the company they are auditing so their reports may not be independent
internal auditors may feel the presure to protect company interest and working relationships
in most of the cases the internal audits are not taken seriously as the external audits
the internal audits judgement can be influenced by the company , afterall they are working on the same organisation

ensure that all the employees are adequately trained in health and safety procedures and aware of their responsibilities and
the consequences of non-compliance
find whether the institution has emergency plans for most predictable emergency
ensure the usage of proper PPE's and safety harnesses
analyse the overall Welfare of the employees
enquire about the previous near miss insurance or accidents if there is any
ensure the risk assessments were updated according to the changes in organisation
