The GLSM Patellofemoral Dislocation [Rehabilitation Program is an evidence-based and soft tissue healing dependent program which allows patients to progress to vocational and sports-related activities as quickly and safely as possible. Individual variations will occur depending on surgical details and patient response to treatment. Avoid ROM with chondrosis or pain when performing OKC knee extension strengthening exercises. Phase 1: 0-2 weeks ROM: Drop lock brace 0-1 wk: 0 1-2wk:0-30 WB: 50 % with brace locked for ambulation Modalities: Cryotherapy 4x/day IFCfor pain/effusion NMES quadriceps RX: Recommendations: Modified Sapega-McClure technique: 1. Heat in stretch: Prone hang 1stTERT TERT= Total End Range Time 2. Mobilization / ROM: PF mobs if indicated - avoid lateral glides 3. Therapeutic exercises: Flexibility exercises: Hams, Gastroc-soleus Biofeedback QS with adductor squeeze, SLR Hip 4 way SLR Gastroc strengthening (50%WB) M<l quadriceps/hamstrings at 0 and 30 deg Short arc 0-30 quadriceps if no chondrosis Balance/proprioception exercises at 50%WB CV conditioning, Core stability Upper body exercises 4. Ice in stretch: 2nd TERT 5. HEP for 3rd TERT Phase II: 2-4 weeks ROM: Drop lock brace 2-3 wks: 0-60 3-4 wks: 0-90 WB: 2-3 wks:' Full WB with brace locked for ambulation 3-4 wks: Open brace for ambulation if good quad control Modalities: Cryotherapy IFC for pain/effusion NMES quadriceps RX: Recommendations: Sapega-McClure technique: 1. Active warm-up: Bike high seat for ROM 2. Heat in stretch: Prone hang 1st TERT 3. Mobilizations / ROM: PF mobs if indicated - avoid lateral glides AROM/AAROM/PROM 4. Therapeutic exercises: Flexibility exercises Biofeedback QS with adductor squeeze, SLR, CKC knee extensions Total leg strengthening Hip 4 way SLR Gastrocsoleus exercises M<l quadriceps/hamstrings at 10, 30, 50,70,90 per ROM Short arc quads/hams per ROM if no chondrosis Hamstring isotonics CKC exercises - leg press w/adductor squeeze, step-ups, mini-squats, partial lunges, lateral step-over Balance/proprioception CV conditioning, Core stability 5. Ice in stretch: 2nd TERT 6. HEP for 3rd TERT

Phase III: 4-6 weeks+ ROM: Patellofemoral brace 4-5 wks: No limitations with progression to full ROM WB: No restrictions Patellofemoral brace Modalities: Cryotherapy NMES quadriceps RX: Recommendations: Sapega-McClure technique if needed (see previous) Bike with resistance Elliptical Runner / Stairmaster Flexibility exercises Biofeedback Total Leg Strengthening Hip strengthening Heel raises Hamstrings isotonics Quadriceps isotonics in ROM without chondrosis Isokinetic quad/hams in ROM without chondrosis CKC exercises- leg press, step-ups, lunges to 90, squats to 90 Baiance/proprioception CV conditioning, Core stability Return to running if 75% strength Plyometrics / Agility exercises / Sport-specific exercises if 75% strength Testing: 6-8 wks Linea / Biodex FXN tests when appropriate Return to Work/Sport No pain or effusion Full ROM Isokinetic Strength- 90% Functional Tests - 90% MD approval
