Safety Comments:
SAFE OBSERVATIONS: -
1- Observed crew members conducting good housekeeping on the rig floor.
2- Observed crew members wearing the right PPE while working near the mud tanks.
3- Observed gas testing was conducted prior to welding activity.
4- Observed while making up the DP with the manual tongs, the crew members got out of the line of fire.
5- Observed the CM raised a PTW & isolation certificate before commencing work of changing out the liner for the mud pump.
6- Observed good hand signals being provided by the crew members to the crane operator while lifting was in progress.

UNSAFE OBSERVATIONS: -
1- Observed a crew member sitting on the mud tank handrails.
2- Observed poor housekeeping near the pipe racks area.
3- Observed the crew member not using appropriate 3-point contact while working on the rig floor.
4- Observed 3rd party forklift operator had not parked the forklift in the designated area.
5- Observed the toolbox and tool basket required better housekeeping.
6- Observed the eye wash station on the mud tanks and rig floor was not functioning properly.

- Bleed of through choke. Open BOP, fill up through annulus with 1.8
m3/min and monitor the well (Ok to continue POOH).

* Conducted safety stand down meeting with crew, appreciated them

for very good and fast response in securing the well, discussed the
risks and mitigations.
Note:
- Observed MM-108 has pressure 3176 Kpa (water injector well which
should be shut in).
- Fill up through annulus with 1.8 m3/min.
