    In reference to DepEd Memorandum OUF-2023-0132 (Revocation of License to Operate of the MTSLAI) dated February 20,
    2023, I am writing this letter to request for the deletion or stoppage of all my deductions from Manila Teachers’ Mutual Aid
    System, Inc. of Solano Nueva Vizcaya Branch to wit:
    CODE DESCRIPTION AMOUNT
    2089A MTMAS-MUTUAL AID BENEFIT 50.00
    2089B MTMAS-SAVINGS BANK 200.00
    I have already settled all my outstanding balances in August, 2022 and have no current loans with MTSLAI. However, I am
    very concerned that recently, MTSLAI deducted Php1,200.00 from my savings deposit without my knowledge and consent. Hence, I
    am requesting your office for the stoppage of the above-mentioned APDS Codes.
    Attached are the photocopy of my latest payslip and my savings ledger with MTSLAI for your reference.
    Thank you very much and I look forward to your favorable action regarding this matter.
