    AFFIDAVIT OF VEHICULAR ACCIDENT

    I, , of legal age, married and with residence address at after having been
    duly sworn to according to law, hereby depose and state:

    1. That I am the registered owner of a motor vehicle which is specifically described as follows:

    2. On the aforementioned date and time, I parked my vehicle at the said parking lot and duly secured
    it. I affirm that at the time of parking, the vehicle was in good condition and free from any
    damages.
    3. After a lapse of approximately [Duration], I returned to the parking lot to retrieve my vehicle.
    Upon reaching my parked location, I was shocked and dismayed to discover that my vehicle had
    sustained damages consistent with a hit-and-run incident. The damages include [Description of

    4. I immediately inspected the surrounding area, but unfortunately, I could not locate any witnesses
    or obtain information regarding the responsible party.
