*King of Freight is a third-party freight brokerage. King of Freight has contracted with EVANNA EXPRESS INC to be the sole motor carrier for the
shipment. EVANNA EXPRESS INC is responsible for making sure it meets all requirements to legally haul this shipment. If the load has additional
requirements, including but not limited to permits, escorts, hazmat certification, and route surveys, these must be obtained by EVANNA EXPRESS
INC prior to picking up the shipment. It is agreed and understood that any duties that would be considered that of a motor carrier are not the
responsibility of King of Freight and EVANNA EXPRESS INC is accepting full responsibility for all duties of a motor carrier for the shipment.
EVANNA EXPRESS INC acknowledges it is bound by the terms of the Ratecon and King of Freight's ('KOF') Motor Carrier/Broker Agreement, both of
which are incorporated herein by reference. As more fully set forth in the Ratecon, EVANNA EXPRESS INC is not allowed to subcontract this load to
another carrier, and it is responsible for any and all claims arising out of the shipment. Upon booking this shipment, EVANNA EXPRESS INC has
provided accurate and complete information to King of Freight, confirming it is a motor carrier in good standing with state and federal agencies that
govern its conduct, it has authority to operate as a motor carrier, it meets all insurance requirements for the shipment, and it is able to safely haul the
commodity listed on the product description.
