AGREEMENT

This Agreement is made at Pune on 5th May 2024

Between

Prime Multispecialty Hospital


Shakuntal Chembers, Near Susgaon Bus – Stop, Sus Pune – 411057 .

And

Everest Fleet Pvt Ltd.

Whereas Everest Fleet Pvt.Ltd. desires to avail of the services of the Hospital for onsite health
assistance, First – Aid, Various medical Investigations, Operative Procedure, Hospitalization and
indoor treatment as and when required by Everest Fleet Pvt Ltd.

NOW THEREFORE, it is hereby agreed by and between Everest Fleet Pvt Ltd and the Prime Multi-
Specialty Hospital,
As under

That during the continuance of this Agreement , The Hospital undertakes the responsibility to
provide all medical Treatment Such as Onsite health assistance, First Aid ,Various medical
Investigations , Operative Procedures , Hospitalization and Indoor treatment.

The charges for Onsite health assistance, First-Aid Treatment, Hospitalization, Treatment,
investigation and operative procedures provided by The Hospital to Everest Fleet Pvt Ltd
Is as per this agreement. The Scheduled of charges of the Hospital.

1. Discount Structure: The below Corporate Discount Structure will be applicable to Everest
Fleet Pvt. Ltd. Beneficiaries [Workers, dependents (Officers, Employees)].
a. Registration: Free
b. For OPD at the Hospital:

10% Discount for Consultation on out-patient department.

c. For IPD:

10% Discount on In Patient department (Excluding Consumables, implants, Packages, Medicines


&Lab / Radiology
Google pay /CARD /Cash payment by Students / Employees and Dependents.

Admin charges will be waived off for beneficiaries and dependents.

Self-payment by Students / employees / dependents.

d. For Health Check Up:

15 % Discount on Health Check-up packages.

 This term & condition will be valid only after showing ID PROOF/ VALID EMAIL from
respective authority :

e. The discounts mentioned out under clause- ‘b, c & e’ shall not be applicable for Patients
who are covered under the Medical Insurance Policy/TPA.
f. Charges for any special tests or diagnostic imaging done outside the Meduraa Hospital will
be charged at actual and levied separately.

2. The patient should present their student id card while visiting the hospital for consultant /
treatment in order to avail the corporate discount under this Agreement.
3. If any student would require OPD Services, Responsible person assigned from Everest Fleet
Pvt. Ltd. will call Relationship Manager of Prime Hospital who would fix up the appointment
and confirm back to Everest Fleet Pvt. Ltd. Lectures and health camps will be conducted after
mutual discussions.
4. Need in a year at least two medical checkup camp.

7. For all emergency cases, Hospital will start treatment immediately and inform Everest Fleet
Pvt. Ltd. within 24 hours through email and telephone.
8. Prior appointment has to be sought by the patient for specialist consultation.
9. In Case of Hospitalized Patients –

It’s clearly understood between the parties to this Agreement that Everest Fleet Pvt. Ltd. shall
not be held liable for payment of any medicine / hospitalization bill of any its employees / workers /
and /or their beneficiaries. The concerned employee/workers shall be responsible and liable, in his
sole capacity, for settlement of all bills. The hospital shall not file any claims against Everest Fleet
Pvt. Ltd. for payment of medical/hospitalization bills of any Everest Fleet Pvt. Ltd. employee, unless
an Everest Fleet Pvt. Ltd. authorized representative has specifically issued a credit letter/email to
the hospital for hospitalization of the employee/employee dependent and confirmed in writing that
Everest Fleet Pvt. Ltd. is undertaking the responsibility of settlement of medical bills of the
concerned employee / students /dependents, in which case Everest Fleet Pvt. Ltd. shall clear the
payment within the allowed credit period of 30 days from the date of submission of invoice. In case
there is no such credit letter issued by Everest Fleet Pvt. Ltd. the hospitalization/medical bills shall
be settled by the concerned employee directly.

10. This agreement shall come into force from and shall remain in force for the period of Two
Years.

TERMINATION

This Agreement shall be valid for a period of Two years with effect from 12th Nov 2024 to till 12 th
Nov 2026 unless terminated in accordance with the terms of this Agreement.

• If the other party commits a breach of any of its obligations under, this Agreement which is not
remedied within thirty (30) days from the date of giving written notice requiring such breach to be
remedied; or

• By either party, immediately and without notice, if the other party becomes insolvent, makes
any assignment for the benefit of its creditors, or is place in receivership, liquidation or bankruptcy;
or

• By either party by giving one calendar month notice without assigning any reason whatsoever.

IN WITNESS WHEREOF the parties hereunder have signed this agreement on 12th Nov 2024 at Pune.
