LIPOMA

Benign soft tissue tumor


• It’s a benign soft tissue tumor composed of
mature adipocytes enclosed in a capule of
connective tissue
• Most common soft tissue tumor
Location / Site:
• Usually subcutaneous in location , often found
in these sites
• Neck
• Shoulder
• Back
• Trunk
• Proximal extremeties
Causes :
• In 50-60% cases there’s evidence of cytogenic
abnormalities ( rearrangement in chromosome
12 – most common )
• Hereditary conditions ; familial multiple
lipomatosis
• Gardeners syndrome
• Dercum’s syndrome
• Minor injuries ; controversial
Classification
• They’re mostly found in 40-60 age group , but
can also affect young adults and children
• Based on location :
• Intramuscular
• Synovial lipomatosis
• Inter muscular
• Peri/para osteal
• Intradural etc
Morphology
• Gross : Subcutaneous lipoma
• Yellow, Soft well circumscribed lobulated
tumor .
• They have a thin capsule ( presence of capsule
differnenciates it from subcut fat )
• Microscopic :
• Mature adipocytes with small eccentrically
placed nuclei
• Fibrous septa traversing through adipocytes .
Liposarcoma

• Chondroid lipoma
• Angiolipoma
• Fibrolipoma
• Myelolipoma
• Ossifying Lipoma
• Myxolipoma
• Spindle cell lipoma
• Pleomorphic Lipoma
Clinical features
• 1-6 cms
• Slip sign
• Donot exhibit differential warmth
• Painless , soft , non tender
Complications
• Myxomatoud degeneration
• Saponification
• Calcification
• Infection
• Ulceration
• Malignant transformation ; though rare .
