Corrective Action Report

Incident
Location Road 60-02 near section 7 road crossing
Reported by Abdulwahed Otain

Observations Observed that some bricks are laying on the top of MV


cables near the trench area. To avoid the risk of metal
falling down from the top remove the unwanted materials
from site.

This may cause the damage of MV cables at site.

Corrective Actions
Immediate Removal of Debris: All bricks and any other unwanted materials present on top of or near the
MV cables immediately removed by the site team to prevent any potential damage to the cables.
Preventive Barriers: Erected proper barriers and signage around the trench area to prevent any future
material storage or accidental dropping of items near the MV cables.
