    CONTRACT OF SERVICE
    This Contract of Service is entered into by and between MARELIN V. LUBIS, School Principal
    III, Department of Education, Dagatan Elementary School, Division of Lipa City known as the
    Contracting Party and BERLYN A. FAMILARAN, a resident of Brgy. Dagatan , Lipa City,
    known as the Contracted Party:

    WHEREAS, the contracting party shall be responsible for the following:

    1. Pays the contracted party the amount of twelve thousand pesos (Php12,000.00)
    per month.
    2. Monitors and evaluates the output of the contracted party.
    3. Pre-terminates the contract anytime the contracted party fails to comply and
    meet the required output and/or when his services are no longer needed.

    Responsibilities of the Contracted Party:

    1. Implements the mandated school programs, projects and activities.


    2. Coordinates the teaching and non-teaching personnel on cascading the
    mandated programs, projects and activities.
    3. Facilitates the conduct of the training needs assessment in school.
    4. Serves as the DRRM Focal Person and facilitate the implementation of DRRM
    policies, plans and programs.
    5. Serves as the Canteen/Feeding Program Coordinator and facilitate the
    implementation of the related school programs.
    6. Facilitates rendering of services in the school library and school clinic.
    7. Performs other related duties when necessary

    WHEREAS, this contract shall cease upon completion of the job within six (6) months.
    However, the Contracting Party shall have the right to pre-terminate the contract should the
    contracted party fail to meet the required output. Likewise, she has the option to pre-
    terminate the contract should any of the provisions stated- be violated, and for the violation
    of administrative rules and regulations of DepEd, Dagatan Elementary School.

    WHEREAS, the services rendered are not considered accredited as government services.

    WHEREAS, the services rendered are not entitled to benefits enjoyed by government
    employees such as PERA, ACA, and RATA.
