KNOW ALL MEN BY THESE PRESENTS:
That I, _______________ Arellano, of legal age, married, Filipino Citizen and
resident of Brgy. ______________, Baguio City, do hereby depose and state that:
1. That our late father owned several parcels of land located in Brgy. Lettac, San
Gabriel, La Union;
2. That all of parcels of land of our late father were already identified and
unofficially divided between us Heirs;
3. That one thereof is identified as Lot No. 2970, Cad. 972-D;
4. Said parcel of land is under Tax Declaration No. __________ in the name of
our late father;
5. However, when we verified the said subject land to PENRO La Union, the
Survey Claimant listed is the undersigned;
6. That the truth and in fact said subject land belongs to my
brother_______________;
7. That we are requesting the good office of PENRO La Union to correct the said
mistake-drop my name as Survey Claimant instead my brother be listed
__________________. Copy of the final Extrajudicial Settlement of Estate is
hereby attached;
8. That I am executing this affidavit request freely and voluntarily in order to
attest to the truth of the foregoing and said request be hereby granted.
