The purpose of this Action Plan is to outline a structured approach aimed at effectively addressing and
resolving critical issues within the community of Tikiw, located in San Antonio, Nueva Ecija. The plan is
designed to provide a clear pathway for implementing targeted strategies and initiatives that will lead to positive
and sustainable changes, ultimately enhancing the overall quality of life for residents.

Tikiw, situated in San Antonio, Nueva Ecija, faces a range of challenges that have a significant impact
on the well-being and development of its residents. These challenges include inadequate infrastructure such as
poverty, limited access to essential services like healthcare and education, and environmental concerns like
pollution. Addressing these issues requires a comprehensive understanding of the community's needs and
priorities.

The scope of this Action Plan encompasses a wide range of activities and initiatives aimed at addressing
the identified challenges in Tikiw. These activities include conducting thorough assessments of infrastructure
needs, engaging with stakeholders to gather input and support, mobilizing resources effectively, and
implementing targeted interventions to address specific issues. The primary objectives of the plan are to
improve community skills and build community resilience and development. Each objective is tied to specific

Community Profile

The community has an estimated population ranging between 2,500 to 2,600 residents. The gender
distribution in the community shows that approximately 63% are boys and 37% are girls. The age groups within
the community vary, with a significant portion falling within the 35 to 45+ range. There are no particular ethnic
or cultural groups that make up a significant portion of the community.

In terms of socio-economic data, the average household income in the community is 500php a day. The
main sources of income for community members primarily come from farming activities. However, there are
some income disparities within the community, particularly among those who have completed only elementary
education. The level of educational attainment among community members is varied, and there is a lack of
access to vitamins and other basic needs. Despite these challenges, there are existing employment opportunities
within the community, particularly focused on addressing children's health and needs.
One of the major challenges faced by the community is childrens' healthcare and needs. This highlights
the need for efforts to strengthen and ensure the safety and well-being of community members. Understanding
the community profile provides a foundation for addressing these key challenges and embracing opportunities
for growth and development.

Our vision is to create a community where children have access to adequate healthcare, education, and
basic needs, where families are empowered to provide a safe and nurturing environment for their children to
grow and thrive.

We have developed SMART goals to address the main problem of children's healthcare and needs in the
community of Tikiw, San Antonio, Nueva Ecija.

1. To improve children's healthcare: Within the next year, we aim to increase the number of children receiving
adequate healthcare by 50%. To achieve this, we will collaborate with local healthcare organizations to provide
regular health check-ups and vaccinations for children in the community. We will measure our progress and
assess the impact of this initiative after one year.

2. To enhance access to education: Our goal is to increase the number of children attending school by 25%
within the next year. To make this happen, we will work closely with community leaders and educators to
