III. PRESENTED PROBLEM:
Mr. Zeynep Ibrahim with household i.d number 103518008-0229-00003 is found to
have an alleged duplicate outside region (Brgy. Macadar, Kapai, Lanao del Sur) to the household of Zeynep Ibrahim with household i.d. number 072214004-8213-00005.
IV. BACKGROUND INFORMATION:
A. The family
Samer and his wife Salimah are blessed with five (5) children, among these children are Sahraman, Sahara, Sahrodin, Sahawi and Sahrani, these children of Salimah and Samer are enrolled currently studying to tehir respective schools. Saidali is a farmer and in at the same time a constructions worker who earns 15 thousand pesos (not fix) while Salimah is a home maker who earns 3,000 for her gardenig (not fix).
B. Environment/ Community
The family is living in Barangay Campong, Pantar Lanao del Norte. They are living in a peaceful community. Their barangay road is accessible because it consist of two lane roads but it will take 15 to 20 minutes of travel from the national highway. V. ASSESSMENT:
The household of Zeynep Ibrahim is an active beneficiary from barangay Bangcal,
Pantar, Lanao del Norte under set 1A. They have been active in the program for more than 10 years and by far the household still continues to provide the conditions of the program. Recently, the household was found to have an alleged duplicate outside region particularly at barangay Macadar, Kapai, Lanao del Sur. The case manager directly paid a home visitation and informed the household. Mr. Zeynep promptly deny the alleged duplicate and that he has no knowledge about it.
The case manager contacted the assigned Municipal link at barangay macadar, Kapai, Lanao del Sur and found out that the alleged duplicate is under set 5A and is active beneficiary, however the household is not compliant both in FDS and Education and all his monitored children has no school certificates presented and cannot provide any legal documents as stated by the Municipal Link.
The Case manager have printed the payment history of the household 103523008- 0229-0003 and it had reached more than P5,000 grant received as of P4 2024.
Amid the case, the household of Zeynep Ibrahim with household i.d 103523008- 0229-0003 strongly denies the said duplicate, the case manager has decided to retain the household since the grantee have presented all the necessary documents like PSA birth certificates of the household, School Certificate of the children, and other important legal documents.

VII. RECOMMENDATION:
With the assessment above, the undersigned recommends that the household with an household i.d number 103523008-0229-0003 should be retain in the program and the household 072214004-8213-00005 should be delisted since it’s not active and do not have legal documents to present as evidences to own the 4p’s account.
Furthermore, the household 072214004-8213-00005 is a misrepresented by other
person, hence, subject for deletion.
