Mr Jones, I'm sorry to interrupt you but I'm Ben Brown, one of the students attached to Dr Morrison's firm. I've been asked to come and talk to you about the problem that brought you into hospital so that I can tell Dr Morrison about you when he does his ward round later this week. I can spend about 40 minutes talking with you. Is that alright ? Yes sure - go ahead. Well, I'd like to take some notes so that I can write up your history later. How do you feel about that ? That's fine by me.

Often you will be asked to 'take a history from the patient' - an expression that implies that the process flows in one direction, from patient to doctor But we have seen that what the doctor does (e.g. body language, manner of questioning and listening) influences how patients divulge their problems. It has been said that doctors should learn to receive, not to take, a medical history. Remember that you will obtain a more accurate and relevant history of the patient's problems if you develop good communication skills.

You should try to find out their name before the interview. You may prefer to gather the rest of the information during the interview : this avoids asking a list of questions, which may not be a good way to begin an interview.

Sometimes a patient will provide a diagnosis rather than a symptom: 'I have arthritis in my legs
When this happens, it is important to ask the patient what they are experiencing: 'Could you tell me what symptoms you have?'
Don't be put off if the patient provides a diagnosis you have never heard of :
Well, I have Osler-Rendu-Weber disease. I'm afraid I don't know much about that disease yet - perhaps you could tell me,how it troubles you.
The patient may well describe not only their problems but also tell you about their condition remember that patients can teach us a great deal.
DR BUCK That must have been very frightening. Do you want to talk about it now or shall we go on and talk about it later?
Next, ask the patient if they have any other problems It is often easier to identify all their problems before obtaining more information about the presenting complaint :
DR KITE You've told me that you have had trouble passing water. Before we discuss that further, could you tell me if you have any other problems? MR BROWN Yes, I've been a bit unsteady on my feet recently and a bit short of breath when climbing stairs, and I'm worried about my wife who has just had a stroke.
It is important to acknowledge and respond to these emotions before continuing, even if it is decided to deal with the issues later on in the interview.
Make a list of all the patient's problems - physical, psychological and social - to be dealt with in turn. This will help you to structure and pace the interview. It will also decrease the chance of the patient producing a problem (often the one that is worrying them most) just as they are about to leave the room.
In this situation, you should never ignore the problem raised. The strategy you use to deal with it will depend on a number of factors, including the time available and how severe you think the problem is. You should give the patient an opportunity to describe the problem. If you feel it can be discussed at another time, you might say, Perhaps we can deal with that when you come to see me again tomorrow.
Of course, the patient may have problems that they have forgotten or may be reluctant to divulge at the start of the interview.
But as you proceed and build up a rapport, the patient may well feel able to discuss their problems with you.
WHAT : What does it feel like ? What brings it on ? WHERE: Show me where it is. HOW : How bad is it ? How is it altered by food, exertion, etc. ? WHEN : When did it start ? When does it occur ? (How often ? How long for ?) WHY : Why do you think you have got it ? WHO : Who is affected by it ?
timing : when ?
You will need to know when the pain began how it started its duration how often it occurs how it ended Sometimes it is helpful to ask the patient to recall the onset of the problem and describe how it has developed since then. If they have had numerous different episodes, ask them to describe a typical one.
location : where ?
This is particularly important when the patient complains of pain It is a good idea to ask them to point to the site, e.g. if it is abdominal pain, you need to know the exact position in order to make the diagnosis.
radiation : where ?
It is also necessary to find out if the pain radiates and, if so, where the pain spreads to A person with gall bladder disease may experience pain in the upper abdomen and in the right shoulder Someone with a slipped disc may have back pain that spreads down their leg.
Symptoms vary in quality Pain may be described as sharp, dull, tight, throbbing, constant or 'comes and goes The type of pain is important in differential diagnosis A patient with pleurisy will usually complain of chest pain that is sharp when breathing in Someone who has had a heart attack will usually complain of constant 'tight, gripping pain' in the chest
You will want to know the severity of the problem e.g. if pain is mild, moderate or severe Of course, there is great variation in people's tolerance of pain, so it can be useful to ask them to relate the severity of their present pain to a previous experience:

The pain was pretty awful Would you say it is the worst pain you have ever had ? Well, I had pain in my leg when I had sciatica, but this was far worse than that.
Asking about these can provide essential information for the diagnosis and management of the patient's condition : DR BUCK - Mr DAWES When you had your chest pain, did you have any other symptoms at the time? Yes, my heart was racing and I felt a bit short of breath.
This part of the history becomes easier as your knowledge of clinical medicine increases.
DR ROSS You have told me a lot about your abdominal pain and the way it's associated with wind and problems in passing your motions. Could you tell me now about what you are usually doing when the pain comes on. MRS ALTON I've been thinking about that. I never get this problem when I'm on holiday - it seems to happen mostly when I'm busy at work, or when we're expecting visitors at home.
Find out what makes the symptoms worse and what makes them better :
When you have the pain, is there anything that makes it better? It's better when I've passed wind or had my bowels open. How does what you eat affect it? I used to eat lots of vegetables and fruit, but I've cut that down and the pain seems to be a bit better. Do you ever take any medicines for the pain? No. I tried paracetamol once, but that didn't seem to make any difference.
The best way to obtain a history of the presenting complaint is to start with an open question, note which aspects of the problem the patient describes spontaneously, and then fill in the gaps with closed or probing questions. You may find this difficult to do initially, but it is a skill that comes with practice.
I'm wondering what effect your symptom have had on your life in general. I'm not sure what you're getting at - but I know that they've made me feel a bit low at times A bit low ? Can you explain what you mean by that ? Well, I feel a bit fed up when the pain comes just before we're going out and I have to sit on the toilet for ages and then we're late - myhusband gets really cross. Does that worry you ? Perhaps you could tell me how you are getting on with your husband ? We've had our ups and downs - but not too bad, really. And what about the rest of the family ? Oh, my daughters are fine - we get on really well.
Note how DR ROSS uses open questions, asks her to clarify what she means by 'a bit low', and picks up the verbal cues about her relationship with her husband.
What is the patient's understanding of their problem ? Try to explore what the problem means for the patient - what they think it might be caused by, or related to : "Perhaps you could tell me what you think is causing your problem?" "What concerns you most about your problem?"
It is important to get an understanding of the patient's concerns and how they interpret their symptoms so that you can provide explanations and advice appropriate to their needs. This may be particularly important when a patient has a different cultural background to your own. At this stage, try to summaries what the patient has told you about the present problem so that they can check the accuracy and fill in any gaps.
This section of the interview involves a series of questions related to each of the body systems The purpose is to elicit important symptoms that the patient may have forgotten or may not have considered significant because they were not related to their presenting complaint. Students often have a problem initially with this section of the interview : firstly, because of difficulty in remembering all the questions to be asked; and secondly, because they are afraid that the patient, who is likely to give negative answers to the majority of questions, may wonder about the questions' relevance.
Use an aide memories : write down the body systems or a small card and refer to it in the interview. Introduce this section of the interview by saying something like: 'I'm now going to ask you a series of questions about common medical problems. This is to make sure we don't miss anything that may be important'.
The list of topics to be covered is shown in Figure 3.4. Examples of questions you might ask include :
Cardiovascular :
Could you tell me if you have any trouble with your heart ? What about chest pain or palpitations ? Do your ankles ever swell ?
Respiratory :
Do you have any problem with your lungs - like shortness of breath or coughing ? Is sputum present ? What colour it is ? Have you ever seen blood in it ?
Genito-urinary :
Do you ever have any problems passing water ? What about pain when you pass it ? Does it have an unusual colour or smell ?
Non-verbal cues are particularly important in the assessment of a patient's mental state. Take particular notice of dress and general appearance. Patients who are demented or who are alcoholdependent often neglect their appearance; the depressed patient may arrive dressed in somber colours. During the interview, take note of the eye contact that the patient makes with you; the depressed patient may be reluctant to look you in the eye. Also note their general behaviour: are they restless, retarded, etc.

You may have noticed abnormalities during the interview; it is important to note them as they are indicators of psychological and neurological functioning. Note both the quantity and quality of the patient's speech. The patient who is depressed may speak in a slow flat tone; the excitable, manic patient may have a rapid, pressurized way of speaking.
Mood
During the interview, you will have gathered information about mood or affect from the patient's behaviour and manner of expression. Do they seem depressed, anxious, agitated, manic or withdrawn ? To explore this further, you can ask general questions such as :
"I'm wondering if you still enjoy life as much as you used to?" "How have you reacted to the problems you've had?" "Perhaps you can tell me if you've felt a bit down (stressed, anxious, excited) recently. "

Yes, they did because they said it was a nasty break OK - so just to summarise what you've told me : you had your appendix out when you were young and, more recently, you broke your leg and had a chest infection, but recovered well on each occasion

There are a number of specific conditions that you may want to exclude from the patient's past medical history at this stage.
These will depend on circumstances but may include tuberculosis, rheumatic fever and diabetes.
Family history
This is important for two reasons First, the patient may be suffering from a genetically determined disease. Second, their concerns about their presenting problems may be related to the experience of other members of the family.
example Mrs Alton may be worried about her bowel symptoms because her father died from cancer of the colon. This is an important piece of information
partly because it will help you to interpret how she presents her symptoms, and also because she has an increased risk of developing cancer of the colon, which is, at least in part, genetically determined.

Taking a family history must be done sensitively. Patients will realise the importance of it if you explain the possible significance of the information. a family history of specific diseases

How do they spend an average day ? What is the structure of their family and how do the members relate to each other ? What is their lifestyle ? Do they have any worries about finance, accommodation, etc.?

Well, let me see - I 'went down to the pub perhaps four times last week and drank about three pints each evening. Thank you, that's very helpful.

The answer is generally 'Yes patients have the right of access to the information that a hospital or general practice holds about them It applies to all paper and electronically held records In exceptional circumstances access may be denied, for example when it is considered that a patient may be harmed by seeing their notes It follows that you should never write anything that you would not wish the patient to see In the future patients may be sent copies of the letters written about them and evidence shows that the majority would welcome this.
Make an aide memoire of the history-taking sequece. Most people need to take notes as they are interviewing. You may decide to make rough notes and to write them up later. Remember to explain why you are taking notes and if it is acceptable. Always maintain intermittent eye contact, and don't give the impression that your notes are more important than what the patient is saying

On ward rounds particularly, the consultant and other medical and nursing staff can learn about the patient's history and the findings of the physical examination and investigation. As a student, you are likely to have had the time to get to know your patient well, to have obtained a history, and to have carried out a physical examination. The presentation of a patient will stimulate discussion on diagnosis and management. It is an important learning opportunity for students. Presenting information to others is a crucial skill to be acquired in training and something you will have to do throughout your professional life.
This rarely happens. The majority of patients are happy to see students and often benefit from the time and support you may be able to give them. Sometimes, however, a patient may have told their story to several students and does not want to do it again. Don't take refusal to see you as a personal affront.
As a general rule, you should not answer questions that patients may ask you about their condition but suggest that they ask their doctor. Clearly, you will have to use your own judgement. If the patient asks you a simple factual question and you are confident of the answer, you might reply, but never be afraid to say, 'I don't know, but I'll ask Dr Peters to discuss this with you', or, 'I don't know. I'll go and look it up'.
Confidentiality of information is an essential part of the patient-doctor relationship and has been enshrined in professional codes of practice since Hippocrates. Patients may feel that it is easier to talk to you as a student, particularly as you may be able to spend more time with them than the rest of the staff Occasionally, a patient may give you information that they ask you to keep confidential. This puts you in a difficult position, particularly if the information is pertinent to their management.
you can help them do this through empathy, perhaps by touching their hand, or using reflective comments :
"I understand that this must be very upsetting for you. " "I can understand why you are so upset." "Perhaps you would like to tell me more about how you feel. "
