Definition

Nursing process: Is a systematic


problem-solving approach toward giving
individualized nursing care

Characteristics Of Nursing
Process
It is a framework for providing nursing care
to individuals, families, and communities.
It is orderly and systematic.
It is interdependent.
It can be used in all setting.

Skills that must the nurse having


to apply nursing process:-

Intellectual skill  As problem solving,


and judgements making.

Interpersonal skills 

As those related to the ability to communicate , listen


and convey interest knowledge and information,
to obtain needed data about the client.

Technical skills 
It is related to methods, procedure and the use
of equipment, to collect data.
The use of different resources of the client ,
family or the community.
These skills foster relationships with the client,

family ,and community members.

Phases Of Nursing Process :-


There are five phases of nursing process:
1. Assessment
2. Diagnosis
3. Planning
4. Implementation
5. evaluation.

1. Assessment:-
Is a systematic collection of data to
determine patient's health status and
potential or actual health problem.
the nurse must gather all relevant information
from the primary source (patient) and
secondary source (family members,
significant others, and health records)

Take care:-
Gathering data takes place through
observing, interviewing, and examining
the client and interpreting laboratory data

During the physical examination ; the


nurse use techniques of inspection,
percussion, palpation, auscultation to
obtain further data.

2. Diagnosis:-
Is a clinical judgment about individual,
family, and community responses to
actual or potential health problem and
provide the basis for selecting nursing
intervention to achieve the outcome.

Nursing diagnosis

The official organization that has assumed


responsibility for developing the taxonomy
of nursing diagnoses and formulating
nursing diagnoses acceptable for study is
the North American Nursing Diagnosis
Association (NANDA).

Difference between nursing and


medical diagnosis

• Medical diagnosis : identifies a specific disease,

surgical procedure, illness Usually does not


change
• Nursing diagnosis : identifies the client’s

response to the disease, surgery, illness ,it can be


changed

Analysis of data, identification of
health problems, risks, and
strengths then formulating
diagnostic statement (Problem and
cause)

3. Planning
Once the nursing diagnoses have
been identified, the planning
component of the nursing process
begins. This phase entails:

1. Assigning priorities to the nursing diagnoses


2. Specifying the immediate, intermediate, and long-
term goals of nursing action
3. Identifying specific nursing interventions
appropriate for attaining the goals
4. Specifying expected outcomes
5. Documenting the nursing diagnoses, collaborative
prob­lems, goals, nursing interventions, and
expected outcomes on the plan of nursing care

4. Implementation
Implementation refers to carrying out the
proposed plan of care.
Implementation includes direct or indirect
execution of the planned interventions.
It is focused on resolving the nursing diagnoses
and collaborative problems and achieving
expected outcomes, thus meeting the patient's
health needs.

5. Evaluation

Evaluation, is the final step of the


nursing process, allows the nurse to
determine the patient's response to the
nursing interventions and the extent to
which the objectives have been achieved.
