A 39-year-old male was admitted to our clinic with symptoms of headache, dizziness, nausea, otalgia, otorrhea, tinnitus, and hearing loss in both ears for 3 weeks. Physical examination revealed edema in the tympanic membrane and external ear canal, and pain by palpation in the mastoid area bilaterally. There was no nystagmus, and the rest of the physical examination was otherwise normal. Temporal bone high resolution computed tomography (CT) showed a lesion causing erosion in the mastoid cortex, tegmen tympani, ossicles, and in the bone covering the sigmoid sinus bilaterally. There was also erosion in the superior semicircular canal and petrous bone on the left side. Cortical mastoidectomy was performed under general anesthesia. Histopathologic examination of the tissue revealed Langerhans cell histiocytosis (LCH). In this paper a case with LCH, presenting with bilateral mastoid involvement which has been rarely reported in the literature, is discussed with the existing literature.

A 64 year-old male patient was admitted to our clinic with the symptoms of hemoptysis, hoarseness, throat tripping, foreign body sensation and respiratory distress. At the laryngeal face of epiglottis by using the indirect laryngoscopy, we determined an alive, green-brown colored foreign body with its free parts extending to the rima glottis. Under general anesthesia without intubation, the foreign body was removed by the help of Magill forceps and diagnosed as a leech. In this report, we present a rare case of living leech at the larynx with a short review of the literature.

Amyloidosis is a clinical disorder caused by extracellular proteinaceous deposits of insoluble abnormal group of fibrils with characteristic microscopic, histochemical, and ultrastructural features. Amyloid is an extracellular amorphous eosinophilic protein, which displays apple-green birefringence with polarized light when stained with Congo red or shows metachromasia with crystal violet or methyl violet dyes. [1-4] Systemic and localized forms are the main clinical categories of the amyloidosis. Systemic amy-loidosis can manifest as immunoglobulin light chain derivative (AL) amyloidosis, which is associated with primary systemic amyloidosis, multiple myeloma, or plasma cell dyscrasias. Amyloid associated (AA) amyloid occurs in patients with chronic inflammatory conditions or neoplastic disease. Familial amyloidosis is a rare manifestation of systemic amyloidosis which accounts for less than 2% of the cases. [5] In humans, more than 20 different, unrelated proteins are known to form amyloid fibrils. Nazolarengoskopi orofarenkse do¤ru uzanan bir sa¤ nazofarengeal kitleyi göstermekteydi. ‹ndirekt larengoskopi sa¤ ventrikül band›ndan kaynaklanan ve sa¤ vokal kordun üzerine do¤ru uzanan bir granülomatöz kitleyi ortaya ç›kard›. Granülomatöz kitle normal larenks mobilitesini bozmuyordu. Yutma zorlu¤u öyküsü yoktu. Kontrastl› bilgisayarl› tomografi nazofarenks ve glotisi tutan düzgün yüzeyli bir kitlenin var oldu¤unu gösterdi. Lezyonun eksizyonel biyopsisi için direkt larengoskopi planland›. Histopatolojik inceleme nazofarenks ve larenks amiloidozunu ortaya ç›kard›. ‹leri de¤erlendirmelerde sistemik amiloidoz saptanmad›. Bu olgu sunumunun amac› son derecede seyrek görülen izole primer nazofarenks ve larenks amiloidozu olgusunu sunmak ve güncel literatür bilgilerini kullanarak tart›flmakt›r.

Temporal kemik kırığı olan hastalarda etyoloji, otolaringolojik semptom ve bulguların radyolojik değerlendirmeleri, tedavi yaklaşımları ve sonuçlar değerlendirildi. GEREÇ VE YÖNTEM Yetmiş yedi temporal kemik kırığı olgusu, yaş, cinsiyet, kırığın yeri, kırığın etyolojisi ve kanlı otore, timpanik membran perforasyonu, serebrospinal otore, işitme kaybı, hemotimpanum, fasiyal ve diğer kraniyal sinir paralizilerinin varlığı ve bilgisayarlı tomografi sonuçları yönünden geriye dönük olarak değerlendirildi. BULGULAR Olgularda kırıkların %55'i trafik kazası sonucu meydana gelmişti ve çoğunluğu erkekti (%76,6). Otolaringolojik bulgular sıklık sırası ile erken dönem iletim tipi işitme kaybı (%65,8), kanlı otore (%61,2), hemotimpanum (%58,5), timpanik membran perforasyonu (%25,6), fasiyal sinir paralizisi (%12,3), serebrospinal otore (%8,5) ve sensörinöral işitme kaybı (%5,4) idi. Kırıkların çoğu petroz (%65,8) ve uzunlamasına tip (%51,2) idi. SONUÇ Bu araştırmada 77 temporal kemik kırığı hastasında sıklık sırası ile otolaringolojik bulguları ve tedavi yaklaşımımızı literatür bulguları ile karşılaştırıp tartıştık. Temporal kırıklarda sistematik bir değerlendirme ve tedavi için bir algoritma oluşturduk. Anahtar Sözcükler: Kafa travması; maksillofasiyal yaralanma; temporal kemik. BACKGROUND We aimed to evaluate the etiologies, otolaryngological features, radiological findings, management strategies, and outcomes of temporal bone fractures. METHODS Seventy-seven temporal bone fracture cases were retrospectively evaluated for age and gender distribution, side of the fracture, etiology of injuries, the presence of blood otorrhea, tympanic membrane perforation, cerebrospinal fluid otorrhea, hearing loss, hemotympanum, and facial or other cranial nerve palsies, and computerized tomography reports. RESULTS Nearly 55% of the cases were caused by traffic accidents and were predominantly male (76.6%). Otolaryngological presentations in order to frequency were early conductive hearing loss (65.8%), blood otorrhea (61.2%), hemotympanum (58.5%), tympanic membrane perforation (25.6%), facial nerve paralysis (12.3%), cerebrospinal fluid otorrhea (8.5%), and sensorineural hearing loss (5.4%). Most of the fractures were petrous (65.8%) and longitudinal type (51.2%). CONCLUSION In this research, otolaryngological findings in order of frequency and treatment approaches were compared with literature findings and discussed in 77 temporal bone fracture cases. We formed a management algorithm for the systematic evaluation and treatment of temporal fractures.

Case reports published in the Extra will be peer reviewed in the usual way and can be cited through digital object identifiers (DOI), as can articles published in the printed edition of the regular Journal. These DOIs are published at the foot of each article. Some abstracting and indexing services, such as Medline, do not currently include such publications in their database. For this reason we have decided to include the abstracts of articles published in Extra in the printed edition of the journal.

SUMMARY Aim: To study the histopathologic effects of systemic cortisone treatment on eosinophilic nasal polyposis. Material and method: Systemic corticosteroid treatments were used in 25 patients with nasal polyposis. An oral dose of 1mg/kg prednisolone was given for the first 5 days, than the steroid was tapered in 17 days by decreasing it's dose 5mg per day Nasal mucosal biopsies taken before and after the treatment were evaluated histopathologically. Results: Following medical treatment, a significant reduction in edema and eosinophilia, and an increase in neutrophilia were detected (p 0.05). Conclusion: Corticosteroid treatments have limited therapeutic effect by histopathologic level on nasal polyposis.

OBJECTIVES To evaluate the efficiency of mastoid antral ventilation tube (MAVT) treatment in recurrent/chronic otitis media with effusion (OME). METHODS 20 OME patients who were unsuccessfully treated with ventilation tube (VT) at least twice, who consented to MAVT and who were followed up at least three years were included in the study group. Control group comprised 10 patients who had the same characteristics and refused to undergo MAVT and underwent VT placement again. Pre-operative and post-operative otomicroscopic, hearing tests, and CT findings were compared between the groups statistically. RESULTS MAVT was placed into 24 ears of 20 patients. In the control group, VT was placed in 13 ears of 10 patients. Postoperatively, in the study group, one tympanic membrane with adhesion and nine membranes with retraction returned to their anatomic positions after MAVT.
