Enhancement of sensory input in the form of repetitive peripheral sensory stimulation (RPSS) can ... more Enhancement of sensory input in the form of repetitive peripheral sensory stimulation (RPSS) can enhance excitability of the motor cortex and upper limb performance. To perform a systematic review and meta-analysis of effects of RPSS compared with control stimulation on improvement of motor outcomes in the upper limb of subjects with stroke. We searched studies published between 1948 and December 2017 and selected 5 studies that provided individual data and applied a specific paradigm of stimulation (trains of 1-ms pulses at 10 Hz, delivered at 1 Hz). Continuous data were analyzed with means and standard deviations of differences in performance before and after active or control interventions. Adverse events were also assessed. Introduction: Failure to accurately estimate energy requirements may result in an impaired recove... more Introduction: Failure to accurately estimate energy requirements may result in an impaired recovery. Overfeeding has been associated with increased carbon dioxide production, respiratory failure, hyperglycemia and fat deposits in the liver, while underfeeding can lead to malnutrition, muscle weakness and impaired immunity. Objective: This study aimed to determine the metabolic profile of infant and preschool children submitted to mechanical ventilation in the ICU. Methods: A prospective study was carried out in a pediatric ICU in Rio de Janeiro that included children aged from 1 month to 6 years submitted to mechanical ventilation from June 2013 to May 2015. Indirect calorimetry was used to obtain resting energy expenditure (REE) and oxygen consumption (VO 2) in the first 48 hours of admission. The predicted basal metabolic rate (PBMR) was calculated using the Schofield equation. The metabolic state of each patient was assigned as hypermetabolic (REE/PBMR >110%), hypometabolic (REE/PBMR <90%) or normal (REE/PBMR 90-110%). The ratio of caloric intake to REE was also calculated and ratios of >1.5 and <0.5 were classified as overfeeding and underfeeding respectively. Results: A total of 35 infants and 17 preschool children were included. The male/female ratio was 34/18. In respect of severity of sepsis, 19 patients had septic shock, 24 had sepsis, five had severe sepsis and four had systemic inflammatory response syndrome. We observed a high incidence of hypometabolism (88.5%) and a low incidence of normal metabolism (7.7%) and hypermetabolism (3.8%). A low value of VO 2 was observed in 46.1% of the patients (VO 2 ≤120 ml/minute/m 2), a normal value in 40.4% (VO 2 >120 to ≤160 ml/minute/m 2) and a high value in only 13.5% of the patients (VO 2 > 160 ml/minute/m 2). Among the 52 included patients, 18 were fasting at the moment of the examination. The ratio of caloric intake to REE for the remaining 34 patients showed 38.2% overfeeding, 11.8% underfeeding and 50.0% normal feeding. Conclusion: Predictive equations do not accurately predict REE in critically ill infants and preschool children, resulting in inadequate feeding. Although hypermetabolism and enhanced energy expenditure are the main clinical features of critical illness in adults, the majority of our patients were found to be hypometabolic which reinforces the need for a different approach between adult and pediatric critically ill patients.

Mendeley is a free software which could help in organising research as a reference manager and also collaborate with other researchers online with the latest research. There is a possibility to import papers and read from anywhere online.
Objectives: Help researchers to select articles utilising Mendeley. Asses data from researchers around the world to find trials.
Search methods: After defining P.I.C.O. And search strategy researchers could import references from many databases as PUBMED, EBSCO and Science Direct and others.
Researchers can create a folder for a systematic review and develop subfolders to insert the results found from any databases. The folders can be shared with other researchers online to decide which paper will continue in the SR.
Mendeley web share references from researchers around the world based on researchers uploads articles on the desktop or the web. The Mendeley group databases have over 30 millions of papers. The utilisation of this resources could increase the possibility to find more trials and include on the SR.
The process to find papers in Mendeley is the same from the other. The Boolean operators AND, OR and AND NOT could be used.
Mendeley group has continuously made improvements on the site and desktop based on researchers users. The use of SR is a promising future. The intention is an open web library.

A utilização da cola biológica em diversos campos da cirurgia continua controversa, apesar do gra... more A utilização da cola biológica em diversos campos da cirurgia continua controversa, apesar do grande número de pesquisas. O objetivo do presente trabalho foi comparar a cicatrização da pele de ratos submetidos à sutura e à aplicação de cola biológica em distintos períodos pós-operatórios. Método: Em 10 ratos machos, foram confeccionados dois retalhos de pele em forma de "U" na região dorsal. Um dos retalhos foi suturado com fio de polipropileno 6-0, enquanto o outro foi fixado com cola biológica à base de gelatina-resorcinol formaldeído (GRF). Metade dos animais (n=5) foi acompanhada por um período de oito dias, enquanto os outros cinco foram estudados durante 21 dias. Avaliaram-se os aspectos macro e microscópio das cicatrizes. Resultados: Após oito dias, a ferida em presença da cola biológica mostrou maior inflamação e reação cicatricial mais exuberante. Contudo, decorridos 21 dias, não houve diferença entre a cicatrização dos retalhos cutâneos em presença de fio de polipropileno ou cola biológica. Conclusão: Os resultados indicam que a cola biológica parece ser uma boa alternativa para a síntese de pele em rato.

To evaluate cardiopulmonary involvement in schistosomiasis mansoni, 246 patients from an endemic ... more To evaluate cardiopulmonary involvement in schistosomiasis mansoni, 246 patients from an endemic area of Brazil were examined; 152 had been previously treated for schistosomiasis. Based on stool examination and/or abdominal ultrasonography, the patients were divided into those with schistosomiasis (69%) and those in whom the disease was not present (31%). M mode measurements were similar in the 2 groups. Pulmonary pressure was measured by Doppler echocardiography; 25% of the subjects had pulmonary hypertension. Those with pulmonary hypertension had a higher prevalence of schistosomiasis (80%) than those without (64%; P=O.O3). No case of car pulmonale was diagnosed by electrocardiography or Doppler echocardiography. The prevalence of pulmonary hypertension correlated neither with periportal fibrosis nor with prior treatment for schistosomiasis.

Obstructive jaundice was associated with bacterial overgrowth in the ileum. This effect may be due to the reduced concentration of bile salts, since dietary supplementation reduced the small bowel aerobic bacterial flora.
