Suggestion

I don't remember how many times this has come up, but it's >=2. Most of the issues I've seen or written using that template put n/a or something like that in those fields. Examples: #6861, #7816, #7817.

Proposal: let's remove those fields for now? If folks spam the template we can always add an equivalent back in.
