Prepare documentation for publication #9

This pull request prepares the documentation for publication, by doing the following:

  • Converted from bookdown to mkdocs.
  • Renamed documentation files to remove zend.authentication. prefix.
  • Re-ordered documentation to push adapters into a subdirectory (and DbTable-related docs into a further subdirectory).
  • Added documentation on credential treatment/callback check DbTable adapters, and removed old DbTable adapter docs.
  • Created separate chapter for identity persistence.
  • Moved adapter interface discussion to adapter chapter intro.
  • Added tables that went AWOL during rst -> markdown conversion.
  • Edited for content and formatting.
  • Added documentation build automation.
