It can be a quick 5-minute or in-depth assessment used to evaluate the status of a patient’s health. 👋 INTRO: Hand hygiene, identify patient, vitals, alert & orientation 🗣 HEAD Mouth, Gums, Tongue: moisture + color Pupils: PERRLA (pupils equal, round, and reactive to light & accommodation) Neck Veins: flat = normal, distended = ⬆️ fluid 🫀 CARDIOVASCULAR Auscultate apical pulse for 1 full minute. Note: rate, rhythm, regularity. ⚠️ Normal HR: 60-100 BPM 🫁 PULMONARY Auscultate anterior/posterior lungs in an S-shaped pattern. Note: depth, rate, sounds ⚠️ Normal RR Rate: 12-20 ⚠️ Normal O2 Sat: 95-100% 🚽 GI/GU Inspect, Auscultate, Palpate.
