Apres une periode de jeune, souvent imposee par une symptomatologie digestive consecutive a une m... more Apres une periode de jeune, souvent imposee par une symptomatologie digestive consecutive a une majoration recente de la consommation d'alcool, les ethyliques peuvent presenter une forme de cetose de jeune particuliere: l'acidocetose alcoolique. Il s'agit d'une pathologie frequente chez ces patients mais souvent meconnue en raison de la correction rapide des desordres engendres, par une simple hydratation et l'apport de glucose. Nous decrivons un cas d'acidocetose alcoolique grave, rappelons la physiopathologie de cette affection et attirons l'attention sur ses consequences parfois redoutables.

Introduction: Congestive heart failure (CHF) is characterized by increased inflammatory and apopt... more Introduction: Congestive heart failure (CHF) is characterized by increased inflammatory and apoptotic processes, while the improvement of the underlying hemodynamics has beneficial effects on both inflammation and apoptosis. We investigated the effects of levosimendan and dobutamine on serum levels of interleukin-6 (IL-6), tumor necrosis factor-a (TNF-a), and apoptotic markers soluble Fas (sFas) and soluble Fas ligand (sFasL) in patients with decompensated CHF. Methods: Forty patients (32 males and 8 females, aged 714-2.1 years, NYHA III-IV, LVEF<30%) with decompensated CHF were randomly allocated into three groups and received a 24-hour intravenous infusion of dobutamine (n=13), levosimendan (n=13) or placebo (n=14). Left ventricular dimensions, ejection fraction and end-systolic wall stress were estimated by echocardiography, while levels of IL-6, TNF-a, sFas and sFasL were determined with ELISA at baseline and immediately after the treatment. All values are expressed as means4-SEM. Results: End-systolic wall stress was significantly decreased in the levosimendan-treated group (from 7824-66 to 6614-72 gr/cm 2, p<0.05) while remained unchanged in dobutamine and placebo groups (from 7374-49 and 7224-49 to 7524-54 and 739-4-50 gr/cm 2 respectively, p=NS for both). Serum levels of IL-6, sFas and sFasL were significantly decreased in the levosimendan-treated group (from 12.74-1.6, 6.94-0.98 and 69.3:1:5.3 to 10.98 pg/ml, 5.9-4-0.99 ng/ml and 60.5:t:5.16 pg/mi respectively, p<0.05 for all) but remained unchanged in dobutamine-treated (from 12.4-1.5, 6.94-0.86 and 66.04-5.3 to 13.2 pg/ml, 7.374-0.97 ng/ml and 66.64-6.51 pg/ml respectively, p=NS for all) and placebo groups (from 10.54-1.1, 6.44-0.58 and 67.24-5.1 to 10.71 pg/ml, 5.734-0.66 ng/ml and 68.04-5.01 pg/ml respectively, p=NS for all). Serum levels of TNF-a remained unchanged in levosimendan and dobutamine-treated groups (from 11.94-1.5 and 12.94-1.7 to 11.4-t-1.91 and 12.54-1.71 pg/ml respectively, p=NS for both) while it was slightly increased in the placebo-treated group (from 12.24-1.8 to 13.14-1.91 pg/ml, p<0.05). Conclusions: Levosimendan seems to be superior than dobutamine in patients with decompensated CHF, since it decreases cardiac wall stress, it depresses the expression of pro-inflammatory cytokines, having also anti-apoptotic properties. Therefore, the beneficial effects of levosimendan on the clinical status of advanced CHF patients, may be a result of its anti-inflammatory and anti-apoptotic properties.

Computed tomographic examination revealed a spontaneous isolated dissection of the superior mesenteric artery and an anevrysm of the coeliac artery caused by the arcuate ligament. Outcome was favorable under conservative medical treatment and a three months follow-up was uneventful. This observation offers the opportunity to present recent insights concerning this pathology.

Its deleterious effects, however, are often ignored. We report a case of tetraparesis associated with chronic excessive consumption of cola. The development of muscle weakness is variable, resulting from a hyperpolarization of excitable membranes. The outcome is most often favorable after potassium supplementation and interruption of the offending beverage consumption. The mechanisms involved are multiple and involve both a leakage of fecal and urinary potassium and a potassium entry into the intracellular compartment. Other mechanisms related to caffeine are also involved. It therefore appears mandatory to assess the consumption of such beverages in the presence of hypokalemia and muscle weakness.

The outcome may be dramatic when the disorder remains unidentified, however early recognition using unspecific exams allows prompt therapy. We report the case of a 14-month-old baby who presented with a sudden access of cyanosis resulting from acute methemoglobinemia. Careful investigation identified contamination of familial food by an excess of nitrates related to the use of weel water from rural location.

Phone cardiopulmonary resuscitation (CPR) assists any cardiac arrest witness, previously trained ... more Phone cardiopulmonary resuscitation (CPR) assists any cardiac arrest witness, previously trained or not, through standardized instructions given by phone. These instructions are intended to guide the bystander in checking for responsiveness, opening the airway and checking for adequate breathing, and to initiate chest compressions in case of apparent death. The "Algorithme Liégeois d'Encadrement à la Réanimation par Té1éphone" (ALERT) is an original phone CPR protocol helping the dispatchers to guide callers during the dispatcher's time window, between the beginning of the call and the Emergency Medical Services (EMS) arrival on scene. Its use has been demonstrated to increase the frequency and performance of bystanders' initial CPR efforts. Such improvement might increase the survival chances of out-of-hospital cardiac arrest victims (OOHCA).

The Emergency Departments being the main gateway to our hospitals, they will often have to admit such patients with their distinct characteristics. These individuals present with a more or less marked geriatric profile; it will be essential to determine their degree of frailty, a factor allowing to predict their survival and future level of autonomy. Evaluation tools have been developed to improve the management of these patients at important moments of their stay in the Emergency Department.

It results that influx patients should be regulated by using a triage tool located at the entrance area. This process has been in development for almost 15 years.We propose a new algorithm of triage (ELISA or Liège Scale of severity index at admission) based on a five-score level of emergency from U1 (emergent) to U5 (non urgent). Such a stratification of the state of emergency is associated with a time-delay for the first medical contact (immediate to 120 min) and a corresponding track for providing optimal care (emergency care unit, B sector or bed, A sector or ambulatory and waiting room) which together fit the sorting for the right patient to the right resources in the right place at the right time. Epiploic appendagitis is the term used to describe the inflammation of an epiploic appendage. The... more Epiploic appendagitis is the term used to describe the inflammation of an epiploic appendage. These small masses of fat distributed along the colon, from the caecum to the recto-sigmoid junction can inflammate by torsion, spontaneously or secondarily with the inflammation of an anatomical structure in the neighbourhood. Symptomatology may mimic retro-caecal appendicitis or diverticulitis and the diagnosis by CT avoids unnecessary surgery or hospitalization. Indeed, under conservative treatment by AINS and analgesics, symptomatology regresses in about five days. In this article, we relate the case of a patient with a typical clinical presentation, to remind the elements of this pathological entity.

Evaluation of the aid of an emergency mobile unit to transfer monitorized patients to a Universit... more Evaluation of the aid of an emergency mobile unit to transfer monitorized patients to a University hospital, in the political context of regional care network offering highly qualified but restricted tertiary area centres, and an open prospective study conducted over the 5 first months in 2006. The call regulation was assessed by the emergency physician of the transfer team and all missions were concluded with an evaluating report. An amount of 197 requests were taken into account from which 80 % were addressed between 8 am and 8 pm. The mean average time interval for missions was 59 minutes (base to base) and the distance covered was 20.7 km as a mean. In essence, indications for medical secondary transfer regarded patients in need for acute coronary care (42.6%), specific intensive care (26.4%) and neurosurgical interventions (19.3%). Pinpointing cardiovascular system dysfunction is often difficult because the clinical signs, or the availability and interpretation of physiological measurements, are unreliable. Often patient-specific information is incomplete or confusing, as it comes from a diverse range of sources, such as invasive and non-invasive pressure measurements, flow rates and electrocardiogram signals. Health professionals therefore rely on intuition and experience to make a "clinical" diagnosis and decide treatment. Sometimes this approach results in multiple therapies being applied until a suitable treatment is found. Computed tomographic pulmonary angiography (CTPA) has progressively been established as a first line test in the APE diagnostic algorithm, but estimation of short term prognosis by this method remains to be explored. Eighty two patients admitted with APE were divided into three groups according to their clinical presentation: pulmonary infarction (n = 21), prominent dyspnoea (n = 29), and circulatory failure (n = 32). CTPA studies included assessment of both pulmonary obstruction index and right heart overload. Haemodynamic evaluation was based on systolic aortic blood pressure, heart rate, and systolic pulmonary arterial pressure obtained non-invasively by echocardiography at the time of diagnosis of pulmonary embolism.
