The aim of this study was to verify how laser phototherapy (LPT) used for oral mucositis could influence xerostomia symptoms and hyposalivation of patients undergoing RT. Patients were divided into two groups: 12 individuals receiving three laser irradiations per week (G1) and 10 patients receiving one laser irradiation per week (G2). A diode laser (660 nm, 6 J/cm(2), 0.24 J, 40 mW) was used until completely healing of the lesions or the end of the RT. At the first and last laser sessions, whole resting and stimulated saliva were collected, and questionnaires were administered. According to Wilcoxon and Student statistical test, xerostomia for G1 was lower than for G2 (p < 0.05), and salivary flow rate was no different before and after RT, except for stimulated collection of G2, which was lower (p < 0.05). The incidence of cervical dentinal hypersensitivity is related to the high number of non-carious ... more The incidence of cervical dentinal hypersensitivity is related to the high number of non-carious cervical lesions. This clinical research was developed in order to evaluate the Nd:YAG laser treatment of cervical dentin hypersensitivity after attempting the removal and control of etiologic factor after two different stimuli. Twenty patients participated in this study in a total of 145 teeth, where 104 received the Nd:YAG laser treatment and 41 remained as control. The results showed that there was statistically significant reduction of hypersensitivity as for the groups that received the treatment with Nd:YAG laser, as for the control teeth. However, the reduction of cervical dentinal hypersensitivity was statistically greater when there was the association of the removal of etiologic factors with the application of Nd:YAG laser. We concluded that the laser irradiation was effective in the treatment of cervical dentin hypersensitivity after 6 months.

The ability of the laser irradiation to promote the cleaning and disinfection of the radicular ca... more The ability of the laser irradiation to promote the cleaning and disinfection of the radicular canal system has become this type of treatment in a viable and real alternative in endodontics. The purpose of this study was to evaluate the apical marginal sealing of root canal fillings after the irradiation with the laser of Nd:YAG or of Er:YAG. Forty-two human, extracted single-rooted teeth had their crowns sectioned and the root canals prepared with a no. 70 K-file. Then, they were dried and divided into three groups according to canal wall treatment: group 1: the canals were filled with EDTA for 3 min, followed by irrigation with 1% sodium hypochlorite solution; group 2: the canal walls were irradiated with Nd:YAG laser; and group 3: the canal walls were irradiated with Er:YAG laser. Afterwards, the root canals were obturated by the lateral condensation technique. The roots were externally waterproof, except in the apical foramen and immersed in 2% methylene blue aqueous solution during 48 hours. The results showed that the largest infiltrations happened in the group 3-Er:YAG (7.3 mm), proceeded by the group 1-EDTA (1.6 mm) and by the group 2-Nd:YAG (0.6 mm). The group Er:YAG differed statistically of the others (p &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05). Severe dental decay and changes in tooth structure have been reported in association with the use... more Severe dental decay and changes in tooth structure have been reported in association with the use of lithium in Psychiatry, but lithium effects on tooth inorganic composition remain unknown. A 30-year-old woman with bipolar disorder, treated with lithium carbonate presented severe dental decay. Dentin samples from lithium and healthy volunteers were collected and submitted to ionic and ultrastructural analysis. Samples from the lithium patient exhibited irregular peritubular walls and the mineral crystals were irregularly arranged in the intertubular dentin. In addition, a decrease in Mg/P/Ca and an increase of Zn concentrations were detected. These data suggest that the severe dental decay and changes in the tooth structure observed for the lithium-treated patient are related to dentin mineral loss and that this pathological condition is different from caries lesions.

Human enamel samples were randomly divided into 9 groups (n = 12): G1-eroded enamel; G2-APF gel; G3-AmF/NaF gel; G4-AmF/SnF2 solution; G5-CO2 laser (λ = 10.6 µm)+APF gel; G6-CO2 laser+AmF/NaF gel; G7-CO2laser+AmF/SnF2solution; G8-CO2 laser; and G9-sound enamel. The CO2 laser parameters were: 0.45 J/cm2; 6 μs; and 128 Hz. After surface treatment, the samples (except from G9) were immersed in 1% citric acid (pH 4.0, 3 min). Surface microhardness was measured at baseline and after surface softening. The data were statistically analyzed by one-way ANOVA and Tukey's tests (p < 0.05). G2 (407.6 ± 37.3) presented the highest mean SMH after softening, followed by G3 (407.5 ± 29.8) and G5 (399.7 ± 32.9). Herpes is a common infectious disease that is caused by human herpesviruses. Several treatments h... more Herpes is a common infectious disease that is caused by human herpesviruses. Several treatments have been proposed, but none of them prevent reactivation of the virus. This article describes the use of photodynamic therapy (PDT) as a treatment for herpes lesions, and reports on four cases. PDT was used as an adjuvant therapy for the treatment of herpes labialis in four patients. A special type of 0.01% (m/V) of methylene blue solution was applied to the vesicular stage of herpesviral disease and the lesions were irradiated with laser energy (wavelength 660 nm, energy density 120 J/cm(2), output power of 40 mW, 2 min per point, 4.8 J of energy/point, at four points). After 24 h the patients returned and phototherapy was repeated with the same equipment, this time with 3.8 J/cm(2) and 15 mW, for a total dose of 0.6 J. The same procedure was repeated 72 h and 1 wk later. Treatment with low-level laser therapy can be considered as an option in the treatment of herpes labialis, and decreases the frequency of vesicle recurrence and provides comfort for patients. No significant acute side effects were noted and the lesions healed rapidly. Treatment of herpes labialis with PDT was effective, had no side effects, and when associated with laser phototherapy, accelerated the healing process.

The purpose of this study was to report removal of gingival melanin pigmentation using an Er:YAG laser in a short-term clinical observation. Five patients with gingival melanin pigmentation participated in this study. Irradiation was performed at 64.0 mJ/pulse (8.5 J/cm2 per pulse) and 10 Hz under water spray in contact mode. Clinical parameters, such as bleeding, swelling, redness, and healing, were evaluated immediately after the surgery and 24 hours, 1 and 4 weeks, and 3 months later. A visual analog scale was used to evaluate the pain level experienced. The Er:YAG laser effectively ablated the epithelial tissue containing melanin pigmentation. At 1 week, gingiva showed fast epithelization with a healthy appearance in all cases. At 2 weeks, gingiva showed satisfactory healing with significant improvement in color and recovery of the tissue thickness. At 1 month, complete healing was observed; after the 3-month evaluation, no gingival deformity or recession was observed. However, there was a slight recurrence in one case. Removal of gingival melanin pigmentation can be performed safely by Er:YAG laser resulting in an esthetically significant improvement of gingival discoloration.

The effects of phototherapy on herpes lesions have been clinically demonstrated by either prevent... more The effects of phototherapy on herpes lesions have been clinically demonstrated by either preventing the lesion formation or speeding their repair. The aim of this in vitro study was analyze the effect of phototherapy on epithelial cells and HSV-1 in culture. Cultures of HSV-1 and epithelial cells (Vero cell line) were used. The irradiations were done using a GaAlAs laser (660 e 780 nm, 4.0 mm2). One, two and three irradiations with 6 h-intervals were done. The experimental groups were: Control: non-irradiated; 660 nm and 3 J/cm2 (2.8 sec); 660 nm and 5 J/cm2 (3.8 sec); 780 nm and 3 J/cm2 (1.9 sec), and 780 nm and 5 J/cm2 (2.5 sec). The HSV-1 cytopatic effect and the cell viability of irradiated cultures and controls were analyzed in four different conditions: irradiation of non-infected epithelial cells; epithelial cells irradiated prior infection; virus irradiated prior infection; irradiation of HSV infected cells. The mitochondrial activity and cytopathic effects were assessed. The number of irradiations influenced the cell growth positively and proportionally, except for the 660 nm/ 3 J/cm2 group. Any variation in cytopathic effects was observed amongst the experimental groups. The viability of infected cells prior irradiation was significantly higher than that of non-irradiated cultures when 2 irradiations were done. Under the experimental conditions of this study we concluded that phototherapy is capable of enhancing epithelial cell growth and prolonging cell viability of HSV-1 infected cells. Positive benefits of phototherapy could be resultant from prolongation of infected cells viability, corroborating with host defenses.
The increasing concern and the search for conservative dental treatments have resulted in the dev... more The increasing concern and the search for conservative dental treatments have resulted in the development of several new technologies. Low and high power lasers can be cited as one of these new technologies. Low power lasers act at cellular level leading to pain reduction, modulation of inflammation, and improvement of tissue healing. High power lasers act by increasing temperature and have the potential to promote microbial reduction and ablation of hard and soft tissues. The clinical application of both low and high power lasers requires specific knowledge concerning laser interaction with biological tissues, so that the correct irradiation protocol can be established. The present case report describes the clinical steps of two metal-ceramic crowns development in a 60-year-old patient. Three different laser wavelengths were applied throughout the treatment with different purposes: Nd:YAG laser (1,064 nm) for dentin decontamination, diode (660 nm) for soft tissue biomodulation, and Er:YAG laser (2,940 nm) for inner ceramic surface conditioning. Lasers were successfully applied in the present case report as coadjutant in the treatment. This coadjutant technology can be a potential tool to assist treatment to reach the final success.

Previous studies have demonstrated the efficacy of Er:YAG laser for dental hard tissue removal and cavity preparation. The treatment of occlusal fissures in noncarious permanent human molars (n = 9) was carried out with Er:YAG laser (KEY Laser II) using handpiece no. 2051, noncontact, focused (12 mm), water spray-cooled, pulse energy 200 mJ, and frequency 2 Hz (group 1), and handpiece no. 2055 with a quartz fiberoptic 50/10, in contact, air cooled, pulse energy setting of 350 mJ and frequency 2 Hz (group 2) and 400 mJ/2 Hz (group 3). The specimens were sectioned, dehydrated in a graded series of aqueous ethanol, dried, and sputtering with gold. Morphological change analysis on occlusal fissures was performed by SEM. Group 1 showed removal of fissure debris and predominantly enamel etching-like patterns, and groups 2 and 3 showed irregular edges, melting, and recrystallization of fissure enamel, with a lava-like structure and bubble-like voids. The results of this in vitro study suggest that the irradiation of fissures by Er:YAG laser using a fiberoptics (contact and air cooled) produced melting and recrystallization of fissures enamel. Further studies are required with different energy parameters and water cooling to evaluate the thermal effects on teeth.

Human enamel samples were randomly divided into 9 groups (n = 12): G1-eroded enamel; G2-APF gel; G3-AmF/NaF gel; G4-AmF/SnF2 solution; G5-CO2 laser (λ = 10.6 µm)+APF gel; G6-CO2 laser+AmF/NaF gel; G7-CO2laser+AmF/SnF2solution; G8-CO2 laser; and G9-sound enamel. The CO2 laser parameters were: 0.45 J/cm2; 6 μs; and 128 Hz. After surface treatment, the samples (except from G9) were immersed in 1% citric acid (pH 4.0, 3 min). Surface microhardness was measured at baseline and after surface softening. The data were statistically analyzed by one-way ANOVA and Tukey's tests (p < 0.05). G2 (407.6 ± 37.3) presented the highest mean SMH after softening, followed by G3 (407.5 ± 29.8) and G5 (399.7 ± 32.9). Herpes is a common infectious disease that is caused by human herpesviruses. Several treatments h... more Herpes is a common infectious disease that is caused by human herpesviruses. Several treatments have been proposed, but none of them prevent reactivation of the virus. This article describes the use of photodynamic therapy (PDT) as a treatment for herpes lesions, and reports on four cases. PDT was used as an adjuvant therapy for the treatment of herpes labialis in four patients. A special type of 0.01% (m/V) of methylene blue solution was applied to the vesicular stage of herpesviral disease and the lesions were irradiated with laser energy (wavelength 660 nm, energy density 120 J/cm(2), output power of 40 mW, 2 min per point, 4.8 J of energy/point, at four points). After 24 h the patients returned and phototherapy was repeated with the same equipment, this time with 3.8 J/cm(2) and 15 mW, for a total dose of 0.6 J. The same procedure was repeated 72 h and 1 wk later. Treatment with low-level laser therapy can be considered as an option in the treatment of herpes labialis, and decreases the frequency of vesicle recurrence and provides comfort for patients. No significant acute side effects were noted and the lesions healed rapidly. Treatment of herpes labialis with PDT was effective, had no side effects, and when associated with laser phototherapy, accelerated the healing process.

The purpose of this study was to report removal of gingival melanin pigmentation using an Er:YAG laser in a short-term clinical observation. Five patients with gingival melanin pigmentation participated in this study. Irradiation was performed at 64.0 mJ/pulse (8.5 J/cm2 per pulse) and 10 Hz under water spray in contact mode. Clinical parameters, such as bleeding, swelling, redness, and healing, were evaluated immediately after the surgery and 24 hours, 1 and 4 weeks, and 3 months later. A visual analog scale was used to evaluate the pain level experienced. The Er:YAG laser effectively ablated the epithelial tissue containing melanin pigmentation. At 1 week, gingiva showed fast epithelization with a healthy appearance in all cases. At 2 weeks, gingiva showed satisfactory healing with significant improvement in color and recovery of the tissue thickness. At 1 month, complete healing was observed; after the 3-month evaluation, no gingival deformity or recession was observed. However, there was a slight recurrence in one case. Removal of gingival melanin pigmentation can be performed safely by Er:YAG laser resulting in an esthetically significant improvement of gingival discoloration.

The effects of phototherapy on herpes lesions have been clinically demonstrated by either prevent... more The effects of phototherapy on herpes lesions have been clinically demonstrated by either preventing the lesion formation or speeding their repair. The aim of this in vitro study was analyze the effect of phototherapy on epithelial cells and HSV-1 in culture. Cultures of HSV-1 and epithelial cells (Vero cell line) were used. The irradiations were done using a GaAlAs laser (660 e 780 nm, 4.0 mm2). One, two and three irradiations with 6 h-intervals were done. The experimental groups were: Control: non-irradiated; 660 nm and 3 J/cm2 (2.8 sec); 660 nm and 5 J/cm2 (3.8 sec); 780 nm and 3 J/cm2 (1.9 sec), and 780 nm and 5 J/cm2 (2.5 sec). The HSV-1 cytopatic effect and the cell viability of irradiated cultures and controls were analyzed in four different conditions: irradiation of non-infected epithelial cells; epithelial cells irradiated prior infection; virus irradiated prior infection; irradiation of HSV infected cells. The mitochondrial activity and cytopathic effects were assessed. The number of irradiations influenced the cell growth positively and proportionally, except for the 660 nm/ 3 J/cm2 group. Any variation in cytopathic effects was observed amongst the experimental groups. The viability of infected cells prior irradiation was significantly higher than that of non-irradiated cultures when 2 irradiations were done. Under the experimental conditions of this study we concluded that phototherapy is capable of enhancing epithelial cell growth and prolonging cell viability of HSV-1 infected cells. Positive benefits of phototherapy could be resultant from prolongation of infected cells viability, corroborating with host defenses.
The increasing concern and the search for conservative dental treatments have resulted in the dev... more The increasing concern and the search for conservative dental treatments have resulted in the development of several new technologies. Low and high power lasers can be cited as one of these new technologies. Low power lasers act at cellular level leading to pain reduction, modulation of inflammation, and improvement of tissue healing. High power lasers act by increasing temperature and have the potential to promote microbial reduction and ablation of hard and soft tissues. The clinical application of both low and high power lasers requires specific knowledge concerning laser interaction with biological tissues, so that the correct irradiation protocol can be established. The present case report describes the clinical steps of two metal-ceramic crowns development in a 60-year-old patient. Three different laser wavelengths were applied throughout the treatment with different purposes: Nd:YAG laser (1,064 nm) for dentin decontamination, diode (660 nm) for soft tissue biomodulation, and Er:YAG laser (2,940 nm) for inner ceramic surface conditioning. Lasers were successfully applied in the present case report as coadjutant in the treatment. This coadjutant technology can be a potential tool to assist treatment to reach the final success.

This study evaluated by scanning electron microscopy (SEM) the morphological changes in occlusal ... more This study evaluated by scanning electron microscopy (SEM) the morphological changes in occlusal fissure enamel, of permanent models, irradiated by Er:YAG laser using contact and noncontact fiberoptics in vitro. Previous studies have demonstrated the efficacy of Er:YAG laser for dental hard tissue removal and cavity preparation. The treatment of occlusal fissures in noncarious permanent human molars (n = 9) was carried out with Er:YAG laser (KEY Laser II) using handpiece no. 2051, noncontact, focused (12 mm), water spray-cooled, pulse energy 200 mJ, and frequency 2 Hz (group 1), and handpiece no. 2055 with a quartz fiberoptic 50/10, in contact, air cooled, pulse energy setting of 350 mJ and frequency 2 Hz (group 2) and 400 mJ/2 Hz (group 3). The specimens were sectioned, dehydrated in a graded series of aqueous ethanol, dried, and sputtering with gold. Morphological change analysis on occlusal fissures was performed by SEM. Group 1 showed removal of fissure debris and predominantly enamel etching-like patterns, and groups 2 and 3 showed irregular edges, melting, and recrystallization of fissure enamel, with a lava-like structure and bubble-like voids. The results of this in vitro study suggest that the irradiation of fissures by Er:YAG laser using a fiberoptics (contact and air cooled) produced melting and recrystallization of fissures enamel. Further studies are required with different energy parameters and water cooling to evaluate the thermal effects on teeth.
