Introduction: Static progressive orthotic devices are efficient in treating contractures. However, current outriggers are unable to keep force transmission and the force application angle (FAA) constant. Purpose of the Study: To evaluate the biomechanical performance of the Isoforce outrigger, a novel extension orthosis. Methods: A hand model was used to measure the required force at the outrigger and FAA, while simulating resolution of different contracture angles. We also tested feasibility in a small patient series. Results: The force required with the Isoforce device never exceeded 2.4 N, and the FAA did not change more than 6. Corresponding figures for the reference devices exceeded 16 N and 20. The 7 patients testing the Isoforce extension device showed an extension deficit that decreased from 40 at baseline to 25 at 6 weeks. They rated the device as very comfortable to wear. Conclusions: Isoforce maintains constant force transmission and FAA throughout the full range of motion, promotes the lengthening of contracted structures, and is comfortable to wear. Introduction Proximal interphalangeal (PIP) joint contractures cause significant impairment of the hand and its function. This disabling condition occurs frequently in patients with Dupuytren disease and after sustaining a fracture. 1,2 Of the orthotic methods available for contracture therapy, static progressive orthoses are the most efficient because they offer better control of the tension applied to the joint than dynamic orthoses and increase the passive range of motion (ROM) faster than other methods. 3 Static progressive orthoses are often used in combination with an outrigger, which serves as a lever arm to ensure that the tension force is applied at an angle of 90 to the finger segment. 4 A force application angle (FAA) of 90 should ideally be maintained to ensure that no compression or tension forces act on the joint. Current outriggers maintain a 90 FAA only over a certain ROM of the affected finger segment. After a large alteration in the angle of the finger with successful treatment, the FAA deviates from 90. Current outrigger systems provide motion different from the finger segment 5 : the finger segment rotates around its joint while most outriggers generate unidirectional linear tension, resulting in this deviation. The problem can be solved by using an outrigger with a joint axis matching the finger joint axis. A dynamic flexion orthosis was developed taking this principle into consideration: the swing traction orthosis has a dynamic tension mechanism pulling in 1 direction with the outrigger
