New graduate nurses’ clinical competence: A mixed methods systematic review

Aim: To appraise and synthesize evidence of empirical studies reporting assessment of new graduat... more Aim: To appraise and synthesize evidence of empirical studies reporting assessment of new graduate nurses' clinical competence in clinical settings. Design: Mixed methods systematic review. Data sources: The search strategy included keywords relevant to: new graduate nurse, clinical competence, and competence assessment. The searched literature databases included CINAHL, MEDLINE, Embase, PsycINFO and Web of Science. The search was limited to full-text papers in English or French, published between 2010 and September 2019. Review Methods: Inclusion criteria were: 1) empirical studies; 2) detailed method and complete results sections; 3) competence assessment in clinical settings; and 4) new graduate nurses (≤ 24 months). Two independent reviewers screened eligible papers, extracted data and used the Mixed Methods Appraisal Tool framework for quality appraisal. Divergences were solved through discussion. Results: 42 papers were included in this review: quantitative (n=31), qualitative (n=7) and mixed methods (n=4). Findings suggest that new graduate nurses exhibit a good or adequate level of competence. Longitudinal studies show a significant increase in competence from 0-6 months, but findings are inconsistent from 6-12 months. Conclusion: There are a multitude of quantitative tools available to measure clinical competence. This suggests a need for a review of their rigor. Impact: No recent reviews comprehensively synthesized the findings from new graduate nurses' clinical competence. This review has found that new graduate nurses' competence has been mostly assessed as good, despite the expectation that they should be more competent. Longitudinal studies did not always show a significant increase in competence. These findings can help nurse educators in providing more support to new graduate nurses throughout the transition period or design improved transition program. This review also identified quantitative tools and qualitative methods that can be used for competence assessment.

Previous reports show heterogeneous levels of competency among them. As a positive association between competency and quality of care in clinical settings has been suggested, it is essential for researchers and clinicians to select valid, reliable, and responsive scales to assess NGNs' competence. However, a systematic evaluation of the measurement properties of scales measuring NGNs' competence had yet to be published. Objective: To analyse, evaluate and synthesize the measurement properties of scales used to assess NGNs' clinical competence. Design: A systematic psychometric review based on the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) methods. Data sources: The search strategy included a combination of keywords and thesaurus terms related to new graduate nurses, clinical competence, and competence assessment. Five databases were searched: Embase, CINAHL, MEDLINE, PsycINFO and Web of Science. The search was limited to full-text papers published in English or French, from 2010 to 2019. Review methods: Two independent reviewers screened eligible papers, extracted data related to validity, reliability, and responsiveness of each scale, and evaluated the quality of their measurement properties as well as risk of bias in their psychometric evaluation. Divergences were solved through discussion. Results: Ten scales were included: eight original scales, one culturally adapted and one modified. Of these scales, 5 have been used by researchers from 2010 to assess competence of NGNs and 5 were newly developed. Most scales are divided into 6 to 8 subscales and use an adjectival scale with either 4, 5 or 7 points. Advancing medical education: connecting interprofessional collaboration and education opportunities with integrative medicine initiatives to build shared learning

Background: Improved teamwork between conventional and complementary medicine (CM) practitioners ... more Background: Improved teamwork between conventional and complementary medicine (CM) practitioners is indicated to achieve effective healthcare. However, little is known about interprofessional collaboration and education in the context of integrative medicine (IM). Methods: This paper reports the findings from a constructivist-grounded theory method study that explored and highlighted Australian medical students' experiences and opportunities for linking interprofessional collaboration and learning in the context of IM. Following ethical approval, in-depth semi-structured interviews were conducted with 30 medical students from 10 medical education faculties across Australian universities. Results: Medical students recognised the importance of interprofessional teamwork between general medical practitioners and CM professionals in patient care and described perspectives of shared responsibilities, profession-specific responsibilities, and collaborative approaches within IM. While students identified that limited interprofessional collaboration currently occurred in the medical curriculum, interprofessional education was considered a means of increasing communication and collaboration between healthcare professionals, helping coordinate effective patient care, and understanding each healthcare team members' professional role and value. Conclusions: The findings suggest that medical curricula should include opportunities for medical students to develop required skills, behaviours, and attitudes for interprofessional collaboration and interprofessional education within the context of IM. While this is a qualitative study that reflects theoretical saturation from a selected cohort of medical students, the results also point to the importance of including CM professionals within interprofessional collaboration, thus contributing to more person-centred care.

<p>Challenges Faced by Female Healthcare Professionals in the Workforce: A Scoping Review</p&gt

Background: The number of women entering the medical and healthcare workforce globally has increa... more Background: The number of women entering the medical and healthcare workforce globally has increased in the past several decades. Women have many roles and positions in healthcare organizations, hospitals and healthcare education settings. Although there has been an increase in the number of women, female workers continue to face many workplace challenges. This scoping review aimed to explore the challenges female healthcare professionals face in the workforce. Methods: A scoping review utilizing Arksey and O'Malley's six-step framework was undertaken to identify and map available literature addressing challenges faced by female healthcare professionals in the workforce. The databases searched included Embase, EmCare, Medline, Cumulative Index of Nursing and Allied Health Literature (CINAHL) and Business Source Complete (BSC). Additional searches were performed using Google Scholar, Trove and grey literature. Results: The initial search yielded 2455 publications (Medline n=369; EmCare n=276; Embase n=612; CINAHL n=1088; Business Source Complete n=109; mixed grey literature n=1). After removal of duplicates, 1782 citations remained. Abstract and title screening reduced the field to 36 publications, following which full-text reviews were conducted. Consensus was reached on 16 publications for final review. After analyzing the articles, three themes were identified: i) family responsibilities, ii) workplace environment and iii) stereotyping. Conclusion: Findings confirm that female healthcare professionals face circumstances that may affect their family lives, as well as factors relating to the workplace environment and stereotypes. Implementing strategies such as reduced work hours, flexible timing and parttime work can support women in the workplace, which then enhances and supports gender equality in healthcare organizations.

This study 4 aimed to investigate perceptions of EMS experts, leaders, managers, and academics, 5 about the challenges faced by Saudi female paramedics in the Saudi Arabian EMS 6 workforce. 7 8 Design/methodology/approach: 9 A descriptive qualitative research approach was used employing semi-structured face-to-face 10 interviews with seven EMS leaders, managers and academics in Riyadh, Saudi Arabia. Data 11 were analysed using thematic analysis informed by the work of Braun and Clark. 12 13 Findings: 14 Three themes emerged from the interviews that described EMS experts' perceptions, namely, 15 cultural and family challenges, the value of Saudi female paramedics in the workforce, and 16 workforce issues. 17 18 Originality/value: Overall, the interviews revealed that Saudi females faced several challenges 19 that could affect their job duties and capacity to work in the EMS. Currently, Saudi females 20 face difficulties with family, social and religious responsibilities, such as taking care of 21 children and homes. Workforce issues were also considered problems that affect Saudi female 22 paramedics in the workplace, such as physical fitness and psychological burnout. The EMS 23 leaders, managers and academics emphasised that due to the new Saudi 2030 vision, it is 24 expected and recognised that more females need to be employed in the EMS workforce. 25

Informatics capabilities are essential practice attributes. Therefore, measurement of these competencies is important as students transition from education to practice. This rapid review, conducted according to Cochrane and World Health Organization guidelines, examined validity and reliability measurements of existing instruments used to self-assess nursing informatics competencies. MEDLINE, CINAHL, EMBASE, and PubMed databases were searched. Empirical studies with the primary aim of measuring informatics competencies reporting psychometric assessment were included. Methodological quality appraisal involved predetermined criteria based on COnsensus-based Standards for the selection of health Measurement INstruments taxonomy. Data extraction and critical appraisal of six studies from four countries identified five instruments. Study characteristics, constructs measured, population, instrument type, and psychometric properties of interest were examined. There were some similarities and differences with instrumentation. Two important measurement properties, content validity and reliability, were underreported. Gaps in key measurement properties were identified, which require further research. Selection of a practical evaluative instrument to implement should also consider the constructs measured, and practicality and feasibility of the instrument for its suitability in different settings. A larger investigation comparing more informatics competency assessment instruments is necessary for an exhaustive analysis.

Although simulation is a recognized means to learn communication skills, little is known about students' team communication in a simulated setting. This review sought to explore what is known about undergraduate health professionals team communication in simulated emergency settings. The Joanna Briggs Institute framework was used to identify studies through five databases. After titles and abstracts and full texts review, 22 studies were identified for analysis and synthesis using inductive thematic analysis. Three key themes emerged: the context of simulation, teamwork and communication, and student interpretations. Simulation is a successful tool to experience communication and teamwork for students. Non-technical skills enhanced students' performance of clinical skills based on expert rating and in understanding shared values, collaboration, and respect for each other, and improving confidence. Simulation is a suitable approach to measure and enhance team communication and teamwork. Exploration of gender, age, and ethnicity and other factors to assess their impact on communication should be the focus of future research in this area.

Healthcare establishments in Saudi Arabia mirror these trends, including having a low number of female paramedics (which is representative of the whole country in general). Although the paramedic situation has changed in many Western countries, there is still a lopsided gender ratio in the Middle East, especially in the Kingdom of Saudi Arabia. The Saudi government is implementing plans for the country's future in Saudi Vision 2030, which promotes increasing women's empowerment and employment. This paper focuses on developing a theoretical framework using gendered organizations theory (GOT) and glass ceiling theory to explore challenges that limit the participation of female paramedics in the Saudi prehospital setting. The theoretical framework has been developed using literature from previous studies and will assist in exploring challenges that Saudi female paramedics face in the workplace and how these challenges can be reduced or eliminated through policy changes and the provision of equal opportunities in emergency medical services. The framework focuses on the existence of a glass ceiling and GOT in the sector and how they affect the recruitment, education, retention, and work satisfaction of female paramedics.

Background: Peer learning has enabled undergraduate nursing students to develop competence and cl... more Background: Peer learning has enabled undergraduate nursing students to develop competence and clinical skills. It is rare to find quantitative studies examining the effect of peer learning on professional competence development among undergraduate nursing students comprehensively. Objective: To examine the effect of peer learning on professional competence development among Indonesian undergraduate nursing students. Design: A quasi-experimental study with non-equivalent control group pre-test post-test design. Participants: Convenience sampling was used to recruit participants. Seventy-five students completed the study (37 in the intervention group and 38 in the control group). Methods: The intervention group received the peer learning program, while the control group received conventional learning during clinical practice. The Nurse Professional Competence (NPC) was used to collect data at pre-test and post-test measurement. Data were analysed using descriptive and inferential statistics. Results: Professional competence had significantly increased in the intervention group. A significant interaction effect of time (pre-test and post-test) and group on professional competence development was also found. The effect of peer learning on professional competence development was significantly greater than the conventional method. Conclusion: Peer learning was demonstrated as an innovative learning method to develop professional competence during clinical practice among Indonesian undergraduate nursing students. It is recommended for nurse educators to consider implementing peer learning during clinical education.

Background In both academic and clinical settings, the phrase 'clinical credibility' is used when discussing the education of health professionals. No clear definition for the concept is evident in the academic literature. To date, there has not been a study undertaken to develop a working definition of clinical credibility in nursing science. Methods This protocol is stage 1 of a registered report. We will use concept mapping to compare and organise views of three stakeholder groups: Nursing students (n=20) from years 2 or 3 of a three-year Bachelor of Nursing program, Nurses and other health professionals (n=20) still practising within a clinical setting, and people who have recently been inpatients in hospital (n=20). Firstly, participants will take part in separate stakeholder focus groups to generate statements about 'What is clinical credibility?". Secondly, the same participants will complete two tasks: (1) rank the importance of each statement (prioritising), and (2) put the statements that appear to go together into groups, (clustering). The data will then be analysed using a specialised software program called Ariadne. A visual 'concept map' will be produced showing the importance and relationship of each statement as viewed by the participants. The projected timeline for the study is one year. The paper complies with the SPIRIT 2013 guidelines for study protocols adapted for mixed methods research.
