- Holotype: MVZ 12094, adult ♀, skin, skull and postcranial skeleton, collected by E. Despard on 8 July 1910.
- Type locality: “Mt. Douglas (altitude 4,200 feet [1280 m]), twenty miles [32 km] south of Alberni, Vancouver Island, British Columbia”.
