She laments making a new friend who later disowns her for claims of associating with other girls she did like. To April this sounded horrible as it made her develop suicidal thoughts. After expressing her worries, Jennifer asked April several questions to determine the sincerity of her suicidal thoughts. Based on her responses, Jennifer concluded that she was not suicidal and invited her if she seriously gets the suicidal thoughts. In the next session if April returns with sincere suicidal thoughts, Jennifer might come up with a therapy or treatment plan to help April overcome and recover from the suicide thoughts. She might also investigate the cause of such since April might be hiding some important information from her. QUESTIONS Documenting what you did and said during a counselling session is important as it helps you make some recommendations to the client and it also aids in instituting an approach for the coming session. Additionally, keep a record your client’s response to what you say and do, this helps determine if the client is likely to respond positively or negatively to the therapy you are planning to administer. Jennifer should have written the notes of what transpired during the counselling session. This helps capture every important detail because recording them later after the session might have some errors, she might omit some crucial details. Documenting immediately after a counselling session is important for self-protection because under such circumstances the client might commit suicide. People might link her actions are as a result of the counselling session she attended. If the counsellor had documented the discussion during the session she will use them for self-protection.
