(12) reported that in gout patients with CKD, starting allopurinol at a low dose and titrating to a
urate target of < 6.0 mg/dL is a safe and effective approach in CKD populations.
Urate crystals cause the release of inflammatory chemicals called cytokines that can cause pain, swelling, and joint damage.
Above a certain threshold, a fraction of soluble monosodium
urate crystallizes and accumulates in the synovial fluid or other soft tissues; this leads to gouty arthritis attacks and chronic gout and joint damage.
"I think the message we would like to give to clinicians is, 'If you can do that [i.e., maintain the serum
urate level below 6 mg/dL], do it.
In addition, medication to lower
urate levels in people with gout did not appear to benefit or adversely affect the long-term risk of fractures.
The administration of aqueous leaf extract of Manilkara zapota for
urate lowering properties on 25 10-16-week-old male albino mice for 28 days showed that the uric acid levels among the groups treated with the leaf extract and ascorbic acid have demonstrated lowered uric acid levels after hyperuricemia induction by KBr[O.sub.3].
"This study indicates that even for people who have had only one or two prior gout flares, urate-lowering therapy to reduce serum
urate may have benefit in reducing future flares," says lead author rheumatologist Nicola Dalbeth, MD, University of Auckland, New Zealand.
Left untreated, gout can lead to more severe conditions, including recurrent gout, which can erode and eventually destroy a joint; advanced gout, leading to nodules of
urate crystals on fingers, hands, feet, elbows or Achilles tendons; or kidneys tones, which develop from
urate crystals collecting in the urinary tract.
Gout, referred to as the
urate crystal deposition disease, is a painful inflammatory arthritis that can drop the quality of life of affected individuals and restrict them to a wheelchair if not treated on time.
According to epidemiological data, HUA has previously been defined as a fasting serum
urate level >420 [micro]mol/L in males and >360 [micro]mol/L in females measured on two separate days after a normal purine diet.
The
urate connection: MGH researcher Michael Schwartzchild, MD, PhD, is leading a phase-3 trial of a supplement that appears to slow PD progression by elevating blood levels of
urate, a salt produced by the body that is derived from uric acid and acts as an antioxidant.
Urate, with joint aspiration to identify
urate crystals and culture to exclude septic arthritis.
Urate, the final breakdown product of dietary and cellular purine metabolism, is considered a powerful antioxidant and the main contributor to the total antioxidant capacity of plasma (1-3).
