    INSTRUCTIONS
    LIST OF DOCUMENTS REQUIRED TO BE ENCLOSED WITH THE APPLICATION FORM

    The eligibility of the candidates shall be scrutinized on the basis of certificates of these
    requirements submitted with the application form which will be treated as final. The training
    (including rotations), should be completed one month before the date of theory examination.

    Attested Photocopies to be enclosed


    All attestation must be stamped with name and designation of the attester by ONE of the following

    Fellow of the College of Physicians & Surgeons Pakistan with his Fellowship Number.
    Principal/Professor of Medical Colleges/Postgraduate Medical Institutions.
    Medical Superintendent / Head of the Medical Institution.

    2.1

    Certificates of residency training (including rotations) as per prospectus signed by


    supervisor/countersigned by the Head of Unit/ Institution on the letterhead of the institute.
    a) All the Training Certificates must be on proper letterheads of department / institute
    giving dates of starting and ending residency training in each specialty / subspeciality and stamped with official seal, showing name of signing authority.
    b) Only those certificates of residency and other certificates will be accepted by the
    College for the purpose of eligibility, which will be issued by the Supervisor, Head of
    the Unit and countersigned by Head of the recognized institution. Attention of the
    candidates is drawn to these requirements in the application form.

    2.2

    Optic Refraction Module certificate for Ophthalmology candidates only.

    2.3

    All fresh candidates have to submit a certificate of attendance of the required mandatory
    workshops along with application forms:

    Research Methodology, Biostatistics and Medical writing

    c)
    d)

    Communication Skills
    Basic Surgical Skills (For candidates appearing in Surgery & Allied Subjects)

    e)

    Basic Life Support-BLS (Rgistered for training from july-2011)

    MBBS / BDS Degree

    Three coloured photographs taken recently and of prescribed size (5 cm x 6 cm)


    . These
    photographs should indicate name of candidate in capital letters and attested on the back.

    Evidence of having paid examination fee (original Bank Draft /Pay Order)

    Log Book (Prescribed by CPSP) duly signed by concerned supervisor

    Note: Training must have been completed one month before the date of theory examination .
