resident of Camarin, Caloocan City, after having been duly sworn in
accordance with law, hereby depose and state:

1 That I am the brother-in-law of __________;

2 That ________ is the owner of a business known as

3 That _______ conducts business operations at ___________;


4 That I am the owner of real rights over a the above-mentioned
business address of ___________;

5 That I allowed my brother-in-law ____________ to use the


above-mentioned lot as parking space for an SUV without
any compensation. In view of my generous accommodation,
___________ is under no obligation to pay any rental fee
for the use of my property for parking space purposes;

6 As such, I execute this Affidavit in truth for whatever legal purpose


it may serve.
