Department of Nutrition and Dietetics Faculty of Medicine and Health Sciences Universiti Putra Malaysia LOG BOOK CASES GUIDELINES 1. Log Book Cases (LBC) contributes to 10% of the IPD placement. 2. The LBC must be approved once at the end of the training by supervised dietitian. 3. Students are required to document all cases that have been observed or conducted throughout the IPD placement. 4. Format for the LBC is as follow: a. Case index and page (see appendix 1) b. Case checklist and analyses (see appendix 2) c. Nutrition care process (Assessment, Diagnosis, Intervention, Evaluation and Monitoring) d. Any relevant and necessary information related to the case/problem including opinion from the dietitian/ others health care professional, current evidence and practical dietary approach. 5. Students are also required to do a self-reflective analysis of the session that has been conducted or observed. A self-reflective analysis should consist of a clear description of the situation, what you have learned, what you are trying to achieve, analysis of feelings, evaluation of experience, did you act the best way and what are you going to do about it. A self-reflective analysis should not be more than 1 page. 6. All activities including the CME/CDE/ visit to community health centre should be documented properly in the LBC.
