Make a DD of Rs 1100/- for A-category and Rs 2600/- for B1 category OR Rs 2700/- for B2 category from any Bank in favor of Shadan Institute of Medical Sciences If you are applying for A & B1 or A & B2, then make separate DDs respectively.
The Form can be obtained from the College on submission of DD and exhibiting the required Originals. The filled-in original form can be submitted in person/post/email within the cut off date along the two sets of enclosers.
Carry the Original EAMCET-13 H.T and Rank Card, Intermediate Pass Memo, Latest T.C. and show it at the Counter. Submit the DD and TWO Sets of Xerox of all Certificates to obtain the Application Form.
Fill the Application Form legibly and submit with a copy of the following Self-Attested certificates.
i.Copy of EAMCET-13 Hall Ticket & Rank Card if you are applying under A category. ii.Copy of Intermediate / 12th Exam Memo iii.Copy of SSC / Equivalent Exam Memo iv.Latest TC / Migration Certificate v.Copy of the Passport / Driving License / PAN Card / Voter ID as proof of residence of Parents. vi.Bonafide & Character Certificate from the last Institution to be enclosed, which would mention the status of his/her behavioral pattern specially in terms as to whether he/she has displayed persistent violent or aggressive behavior or any desire to harm others. (This is required as per letter No. MCI-34 (1)/2009Med/5191 to 5401 dtd. 30-4-2009). If in case, this is not available at the time of registration, then it must be submitted at the time of admission.
vii For B Category clearly specify by writing on the top of the first page of the Form B1 or. B2. viii. In case of candidates who have studied in other countries shall submit equivalency certificate from Association of Indian Universities, New Delhi and Board of Intermediate Education, Hyderabad. ix. In case of candidates who have studied other than Intermediate Board of Education Andhra Pradesh, shall submit equivalency certificate from Board of Intermediate Education, Hyderabad
On submission of Filled-in application form along with all the enclosures, Registration Slip must be collected. No separate registration charges are to be paid.
If you are submitting the downloaded application form in person then the form number will be issued along with the registration slip.
If you are submitting the downloaded application form by email, the filled-in form and enclosures shall be scanned and sent only toshadan13a@gmail.com for A-category and shadan13b@gmail.com for B1 & B2 categories along with proof of online payment of respective registration charges. Application forms sent to emails other than above will not be accepted. For B Category, clearly specify by writing on the top of the first page of the Form B1 or B2.

If you are sending the downloaded form/ Original Form by post make sure it shall be delivered before cutoff time. No application will be accepted either in-person/by post/ or email after the cut off time.
For B1 and B2 Category the bank guarentee has to be obtained and submitted. Tthe Bank Guarantee shall be in the format of the Nationalised bank with the clause "the Bank shall stand guarantee towards annual tuition and other fees of the candidate till he/ she completes the MBBS course" addressed to the Dean of the College.
