    of 2 references from India
    or USA.

    of any Consulate or

    child/children to be
    removed from your existing
    passport.
    Applicants signature/ in
    case of minor, thumb print
    as per specification.
    Your current city and
    current date.

    Provide both parents details


    in case of Minor applicant
    only.

    In case of Minor
    applicant, both parents
    signature is mandatory.
    Write your current city
    and current date.
    Applicant (minor as
    well) needs to sign
    inside the boxes.
