The candidates are required to submit the following documents at the time of reporting: 1) Self-attested photocopies of documents proving date of birth, educational qualifications, and photo identity. 2) Caste certificate (if applicable) in the prescribed format, with valid non-creamy layer clause for OBC candidates. 3) Medical examination report and fitness certificate from a government civil surgeon certifying the candidate is medically fit.

Instructions to the candidates

The candidates, at the time of reporting are required to submit the following
documents.

1. One self attested set of photo copies in proof of Date of Birth, Educational
Qualifications (S.S.C to the maximum qualification acquired) and photo
identity for verification.

2. Caste Certificate (if applicable) in the prescribed format. In case you belong
to OBC category, the certificate should be in the format as prescribed by the
Government of India and issued by the Competent Authority with date and
Office seal. Name of the caste mentioned in the certificate should tally letter
by letter with the caste name notified by the CENTRAL GOVERNMENT.
In case of OBC Category candidates, ensure that the OBC Caste certificate
should necessarily contain Non-creamy layer clause which should have been
issued within One Year prior to the date of reporting, certifying that the
candidate does not belong to creamy layer section.

3. Medical Examination Report (Medical Fitness Certificate) from a Govt. Civil


Surgeon/Dy. Civil Surgeon, certifying that you are in a fit state of health and
able to perform the duties required by the bank, as per the format enclosed.

4. In case of Persons with Disability (PWD) Category Medical certificate issued


by Competent Authority i.e., Medical Board appointed by the State
Government at District Levels in the prescribed format and (3) i.e.,
Medical Examination Report (Medical Fitness Certificate) from a Govt. Civil
Surgeon/Dy. Civil Surgeon, certifying that you are in a fit state of health and
able to perform the duties required by the bank, as per the format enclosed.

5. Testimonials (Conduct certificates ) issued by two Gazetted Officers in


originals.

6. Declaration Forms (three) & Candidate’s Bio-data Forms (three) for


verification of character and antecedents as per the format enclosed.
