    taking Bachelor of Science in Hotel and Restaurant Management is currently enrolled at LSPU
    Siniloan Campus this A. Y. 2017-2018.

    This waiver demonstrates my willingness to be assigned in MAX’S RESTAURANT INC. and to follow

    Religiously the OJT Policy as follows:

    1. Finish the requested OJT number of hours until the end of the semester.
    2. Discontinue training when no longer enrolled.
    3. Not to establish close relationship to any of the regular employees of the company of
    assignment.
    4. Report any untoward incidents that may occur during the training to the OJT coordinator.
    5. Maintain the good image of the university.

    I promise that I will not make any negative move to disregard the OJT policy for I highly
