a. Participant must be 18 years old and above
b. Participant below 18 years old must secure the parental consent
c. Participants must be checked if they are at the line 10 minutes before the start
d. The participants must put a hand paint on their t-shirt, the moment they reach the color station.
e. Race bib must be worn during the race
f. The decision of organizer is final

ASSUMPTION OF RISK/WAIVER OF LIABILITY AGREEMENT
Hereby declare and state that:

For: COLOR FUN RUN 2016 “Takbo Para sa mga Lider at Manlalarong Tulaynon”

1. I am a participant in the COLOR FUN RUN 2016 “Takbo Para sa mga Lider at Manlalarong Tulaynon” to be held on
September 17, 2016 from Brgy Tulay na Lupa, Covered Court to Mabilo 1, covered Court.
2. I am in good state of health and I am capable of participating in the race. I am aware of all the basic safety and
traffic
rules, and all the inherit danger of FUN RUN, especially it is done on the streets. I am also willing to assume any
and
all risks related hereto. I understand that the race is strictly recreational, and that the race organizers and school
personnel are not there as guardian of my safety;
3. I voluntarily release, discharge, waive, and relinquish any and all causes of action that I and/ or my heirs,
administrators, and assigns may have for any personal injury, property damage, or wrongful death that may occur to
me as a result of participating in the race and any activities incidental hereto, regardless of whether the same shall
arise due to negligence of the organizers of the race. Because of said releases, discharge, waive, and relinquishment,
I or my heirs, administrators, and/or assigns may not prosecute or present any claim for personal injury, property
damage or wrongful death against the organizers/school personnel of the race;
4. I shall furnish my own equipment necessary for the race and I will be solely responsible for its safety and operating
condition;
5. I understand that organizers of the FUN RUN may provide advice and/or assistance that I can follow at my own
risk;
6. I have read and fully understand the rules and regulations of the race, including the rules that the organizers of the
race shall have the final decision on all matters, including but not limited to disqualifications, falls, contact with
other
participants, declaration of winners, cancellation of the race due to the weather, temperature, humidity, traffic and
other conditions, and agree to abide by said rules and regulations and;
7. I authorized the organizers and/or the sponsor of the race to use any information about me with regards to the event.
In connection with the operation of the race or any other events of the organizers and sponsors of the race. I hereby
declare that I have read and understood the foregoing paragraph and fully understand the legal rights that I am
releasing, discharging, waiving, and relinquishing and that these terms, once agreed upon writing, form a binding
contact.
Further, I hereby declare that the information I subscribed herein, particularly my name, age, and address are
true and correct and that I am willing to show proof of the same in case the organizers of the race request such
information.
Finally, I hereby declare that I signed this document of my own free will.
