AUTHORIZATION, WAIVER AND UNDERTAKING

Upon my/our written consent and that of the co-borrower, UCPB may for my/our account, advance the payment of
insurance premiums with respect to any collateral, personal or real property, that shall be mortgaged, pledged or
assigned to UCPB as security for the loan being applied for. I/we, jointly and severally, undertake to reimburse UCPB
for such advances upon demand subject to such interest and charges based on the same interest rate applicable to the
loan, if granted.
I/we hereby authorize UCPB to conduct, through its representative or accredited appraiser, an appraisal of any or all of
the collaterals offered as security for the Loan. We agree that the appraisal report shall be the property of UCPB and
for its sole use only.
In connection with my/our loan application, verification of information, evaluation and/or processing of the loan
(and other credit accommodations) with UCPB, I/we hereby authorize(s) UCPB:
1. To verify with internal revenue authorities and other persons or entities the authenticity of any income or other
tax returns, financial statements, and other credit and financial data and information which I/we have submitted,
or will submit, to UCPB.
2. To inquire into my/our deposits, funds or assets in the custody of UCPB, or its parent, financial subsidiaries or
affiliates, or other persons for the limited purpose of verifying information on my/our said deposits and assets
or implementing any collateral arrangement, assignment of funds, debit authorization or set-off provisions that
may be under the loan and collateral documents.
3. To inquire into, transfer, disclose, submit, share and communicate any credit and financial data and information
of whatever nature, including updates or corrections thereof, pertaining to me/us, as applicant and/or borrower,
fully paid or otherwise, with:
a. Any of UCPB’s offices and branches,
b. UCPB’s parent company, financial subsidiaries and affiliates,
c. UCPB’s agents, counsels, representatives, service providers and other third parties selected by any of them
d. Any credit information bureau, banking or credit industry association, or credit information service provider
and credit/ loan provider, including the Credit Information Corporation (CIC), and
e. Other lenders as authorized by the CIC and credit reporting agencies duly accredited by the CIC.
For purposes of Republic Act No. 9510 and its Implementing Rules and Regulations, I/we hereby acknowledge
that this Application, when signed by me/us, serves as NOTICE duly received by me/us from UCPB that my/our
basic credit data as defined under said law and regulations shall be disclosed as required therein.
To the extent reasonably necessary for UCPB and the data and information recipients to exercise the above rights
and to discharge the functions and responsibilities of the inquiring or disclosing parties and for as long as I/we have
an outstanding loan or credit accommodation with UCPB (or its successors and assigns) and/or as necessary and
required under existing laws, rules and regulations, I/we hereby waive my/our rights under applicable laws on
confidentiality of information or data (such as, but not limited to, Republic Act Nos. 1405, 6426, 8791 and 9510 and
their amendatory laws.)
CERTIFICATION AND SIGNATURE
I/we certify and warrant that all data and information provided in this Application as well as in all supporting or related documents I/we have
submitted to UCPB are true and correct as of date hereof and any information given herein or obtained pursuant to this and under the authority
granted in this Application shall be deemed property of UCPB. I/We agree that in the event any information given herein or in the supporting
or related documents is not true or incorrect, UCPB may reject this Application or cancel any approval if already made and UCPB is under no
obligation to disclose the reason for such disapproval. I/we hereby acknowledge that I/we have read and understood the foregoing
Authorization, Waiver and Undertaking given in this Application and agree to be bound by its terms.
