    JOINT AFFIDAVIT OF COHABITATION

    That we ROSE ANN T. CORNELIA and PAUL WINSTON B. REMOLANO,


    both of legal age, single, Filipinos, and residents of barangay Calo, San Jose, Negros
    Oriental, after having been duly sworn to in accordance with law, do hereby depose and
    manifest:

    That sometime in June 2008, we meet each other in a birthday party of Kurt
    Remolano in Calo, San Jose, Negros Oriental and then we became friends;

    That after several meetings and acquiantances we became intimate friends, and
    moved by our mutual love, and understanding we agreed and decided to live together as
    husband and wife in barangay Calo, San Jose, Negros Oriental in November 2011 until
    the present;

    That in order to put more meaning and make our status legal we have decided to
    marry each other; since we are living together as husband and wife for more than five (5)
    years or for a period of more than Seven (7) years;

    That during our cohabitation we were blessed with one child named, Annton
    Xenia Remolano, who was born in July 21, 2015.

    That this affidavit is executed freely and voluntarily just to attest the truthfulness
    and veracity of my statements herein and not to defraud any government office or
    institution;

    IN WITNESS WHEREOF, I have hereunto affixed my signature this 31 ST day of


    December 2018 at Sibulan, Negros Oriental.
