    ISABELA and parent of MR. LENARD J. SANTIAGO, a student of the College of Engineering,
    Isabela State University – Main Campus, Echague, Isabela, do hereby allow/authorize her to
    undertake on-the-job training outside ISU Campus at DPWH ISABELA 2nd DISTRICT
    ENGINEERING OFFICE, San Antonio, Roxas, Isabela from June to July. I understand that the
    said student will be guided by the University rules on student conduct and pertinent rules and
    regulations of the firm/agency.

    I hereby further agree to shoulder all personal and incidental expenses that may be incurred
    by the student in connection with the fieldwork required for successful completion of her study
    program.
