This is to signify our intention to enroll to the following:
a. Subscribe to the Land Bank of the Philippines’ (LBP’s) internet banking facility known as “weAccess”; and
b. Designate authorized signatory to enroll and designate authorize user/s in the LANDBANK weAccess.
As the School Head/Principal of (CAYANGA ELEMENTARY SCHOOL), I, (BONIFACIO T. OVEJERA)
will act as “Authorizer”, and:
a. to execute/sign all documents necessary to subscribe to LBP’s “weAcess”;
b. to designate (LOWENA ANNE P. FACUN), (DISBURSING OFFICER) to act as “maker” for the company’s access to LBP’s weAccess banking facility; and
c. To ammend/add/delete enrolled deposit account/s as source and/or destination
account/s. access rights ton the weAccess modules, authorization rules and/or any information on the original and succeeding enrollments.
This shall as well, be an addendum to the signatories of the submitted resolution for account opening. Attached is the agency/office enrollment form and other documents supporting our enrollment in your LANDBANK weAccess.
