    AFFIDAVIT OF QUITCLAIM

    I, NAME , of legal age, Chinese, and with office address at No.ADDRESS, having been
    sworn to law, hereby deposes and says:

    That I am the authorized representative of COMPANY a Chinese corporation duly


    organized under the existing laws of the Republic of the Philippines with business address at
    Contractor’s Mailing Address;

    That COMPANY has a Notice to Proceed with National Grid Corporation of the
    Philippines (NGCP) – BID dated ______________.

    That said notice to proceed has been delivered, inspected, turned-over and accepted
    by NGCP;

    That COMPANY has paid all taxes due, obligations incurred, the cost materials, labor
    and equipment rentals, etc. arising from and in relation to the supply and delivery of the said
    order;

    That, in the event of any such Claims, COMPANY hereby holds NGCP free and
    harmless;

    That, COMPANY has no other claims for the said Purchase order against NGCP and
    that we are executing a Quit claim to attest to the fact that COMPANY has no more claims
    except these 10% Retention Billing of the Total Contract Price;

    That COMPANY executes this Quit Claim in connection with the release of this
    Retention money.

    IN WITNESS WHEREOF, I have affixed my signature to this sworn statement on 19th


    July, 2019 at Pasig City, Philippines.
