    That I, TERESA CAPARAZ AMADO, of legal age, Filipino Citizen, and resident
    of #526 GEN. LUNA ST. MALANDAY SAN MATEO RIZAL, after having been duly sworn
    to in accordance with law do hereby depose and say:

    That I will borrow the amount of ONE MILLION PESOS (Php 1,000,000.00) on
    September 01, 2019 from my friend, Dr. ELSA A. TAYHON, of legal age, Filipino Citizen,
    and resident of #315 KABAYANI ST. PARADISE AVE. MALANDAY MARIKINA CITY.

    That I promise to pay her on March 2021 until October 2021 in an amount of ONE
    HUNDRED SIXTY TWO THOUSAND (Php 162,000.00), every month and issued
    postdated cheque on January 2021.

    That as a security of my personal loan from her, I attached the copy of the Transfer
    of Certificate of Title of Land No. 444541 located at Brgy. Dulong Bayang San Mateo
    Rizal:

    That I am executing this affidavit to attest the truth of the circumstances for whatever
    legal purpose it may serve.
