TERMINAL REPORT

In making valid test questionnaires that can be used during examination for ninth graders,
the following persons below indicated their contributions and roles in making this requirement,
for the course Assessment and Evaluation, to be accomplished. In making this all possible, these
students also followed some procedures to be mentioned in the latter part of this paper.

In this course, Assessment and Evaluation, Grade 9 Group (members were mentioned
above) are tasked to make a Table of Specifications for grade 9 in their Mathematics subject.
First and for most, it is really hard to make learning outcomes and decide on what type of
questions the said members to make especially this is only the first time for them to make this.
Fortunately, the group has a Grade 9 Mathematics book anchored to the K-12 Curriculum. They
must do the computations in order to divide and classify the items required if it will belong in the
category of remembering, understanding, applying, analyzing, evaluating or creating. As they
make the Table of Specifications, they have also make errors and the teacher is there to guide
them throughout the process.
After being done, the group is now tasked to construct questions based on the category
and must be aligned on what competencies or learning outcomes they want to achieve in the
making of the questionnaires. The numbers of learning outcomes were distributed to each
member of the group. They must create questions based on their assigned competency/topic and
must also corresponds to the category abovementioned.
The first output is being checked by the teacher and gave back to them for refinement and
corrections. Then, the questionnaires will become a tool in conducting a test in every group’s
assigned grade level.
Before conducting the said test, the group must prepare letters to be given to the OIC of
the school administration (PNU-Mindanao), to the school’s principal in Prosperidad National
High School (where the class planned in conducting the test), and also to the validators of the
questionnaires before proceeding in conducting the test.
After checking the questionnaires by the validators including the Table of Specifications,
the questionnaire is being refined and was narrowed-down from 80 items to 50 items.
Afterwards, the test was conducted on December 10, 2019. The members are grateful to
the Prosperidad National High School in allowing the BME students to conduct the test even
there is a conflict to the schedule. The student-teachers (BME students) are also thankful to the
students who joined in answering the test.
The group abovementioned later do the checking of the papers, tallying, and doing the
item analysis. Then, the results will determine if the members will revise, reject or accept the
questions in every question. The group changes the questions and revises it if needed.
