into valuable health care data to uncover insights and develop strategies for better care at lower costs.

At Optum, our mission is to help people live healthier lives and to help make the health system work
better for everyone. We are tackling the biggest challenges in health care by designing multifaceted
solutions, creating actionable insights and partnering across the entire ecosystem. Together, we are
transforming health care for a better future for everyone we serve.

One of the challenges that we are constantly dealing with is the rising cost of management of chronic
conditions. A number of fundamental structural trends, ageing population and lifestyle changes to name
a few, have created the preconditions for an expansion in the prevalence of chronic disease problems
in the future. Accordingly, the pressure these illnesses are creating on the overstretched health
systems cannot be ignored.

90% of the annual $3.3 trillion health care expenditure in the US is by people
suffering from chronic and mental health conditions. According to the Centers
for Disease Control and Prevention, chronic disease accounts for nearly 75%
of the aggregate health care spending or $5300 per person annually.

As chronic diseases continue to rise in the United States, it is imperative for health care insurance
companies to provide health care services for patients with these costly and long-term conditions.
Chronic diseases have evolved into such an expensive health care issue that experts, such as the
American Medical Association, have asked private and public insurance companies to address it
directly. Insurance companies are now required to fund chronic disease management programs and
other stakeholders have established chronic disease management funds that provide Medicare
beneficiaries financial support to pay for the high costs of chronic care treatment.

Optum’s condition management programs identify at-risk individuals and help resolve health care gaps
by providing the right resources at the right time. Ninety percent of the participants felt the program
helped create a better, more positive health care experience. Our data and analytics allow holistic
viewing and monitoring of total population health. We integrate workflow solutions that deliver data in
real-time and create actionable insights. This helps us identify individuals closer to the time they
experience a change in health, when they are more likely to act and engage. With our expertise, we
enable the patients and health care providers to be equipped to manage chronic conditions. We are
continuously creating strategies to make condition management more efficient and cost effective.

Economic burden of chronic diseases is manifold, imposing costs not only at individual and family level
but at national level as well. Accordingly, disease management can increase quality of life for all the
stakeholders - patients, hospitals and physicians. Patients can become aware about the process of
self-monitoring and managing their conditions. From the physician’s perspective, it is advantageous to
regularly interact with patients and monitoring symptoms. Analyzing the patient’s condition helps
doctors to identify abnormalities before they progress to untreatable levels. Hospitals can also actively
participate in community health management and lower acute diseases.

solutions, adoptable care models, learnings from other industries etc. to address cost burden of these
