    I. ELIGIBILITY AND TECHNICAL DOCUMENTS

    A. ELIGIBILITY DOCUMENTS
    Class “A” Documents
    1. Legal
    1. Valid DTI or SEC Registration
    2. Valid and Current Mayor’s Permit/Municipal License
    3. Tax Clearance Certificate (per Executive Order 398 s. 2005) as finally reviewed and
    approved by BIR
    4. Certificate of PHILGEPS Registration and Membership under Platinum category
    (GPPB circular 03-2016(4.1.2) dated October 27, 2016).
    2. Technical
    1. Statement of all ongoing government and private contracts within the period stated in
    the BDS, including contracts awarded but has not yet started. If none, Certification of
    no ongoing/awarded but has not yet started contracts.

    The statement shall be supported by the following:


    a. Notice of Award and/or Contract
    b. Notice to Proceed
    2. Statement of single largest completed contract (SLCC) similar to the contract to be
    bid within the period stated in the BDS.

    The statement shall be supported by the following:


    a. Notice of Award and/or Contract
    b. Notice to Proceed
    c. End user’s acceptance or official receipt(s) or sales invoice issued for the
    contract or Certificate of Completion/Accomplishment
    3. Financial
    1. Updated Audited Financial Statement, stamped, received by BIR
    2. Latest Income and Business Tax Returns
    3. Net Financial Contracting Capacity (NFCC) Computation which must be at least
    equal to the ABC to be bid or a committed Line of Credit (CLC) from a Universal or
    Commercial Bank

    Class “B” Documents (for joint-ventures)


    1. Valid Joint Venture Agreement (JVA) (if applicable)

    B. TECHNICAL DOCUMENTS

    1. Bid Security in the prescribed form, amount and validity period


    2. Conformity with technical specification, as enumerated and specified in Sections VI
    & VII, which may also include the following and must be signed by the authorized
    signatory:

    b. Manpower requirements for the contract (if applicable)
    c. After-sales service/parts (for equipment) and Warranty (if applicable)
    3. Omnibus Sworn Statement by the bidder as to the required statements provided in
    Section 25.2.a.iv of the Revised IRR of RA 9184 (Notarized) (See Section VIII,
    Bidding Forms)
    4. Other documents (for representative/s) (whichever is/are applicable)
    a.) Special Power of Attorney in case of single proprietorship.
    b.) Resolution from the General Manager or President if partnership.
    c.) Secretary’s Certificate in case of Corporations.

    II. FINANCIAL DOCUMENTS

    1. Duly signed Financial Bid Form (See Section VIII, Bidding Forms)
    2. Duly signed bid prices in the Bill of Quantities
    3. For Goods offered from abroad (See Section VIII, Bidding Forms)
    4. For Goods offered from within the Philippines (See Section VIII, Bidding Forms)

    ANNEXES
    1. Company Profile (Name of Company, Business Address, Telephone Numbers/Fax
    Numbers, E-mail Address)
    2. Authorized Representative of the Company (Name of Representative, Address,
    Mobile Number)
    3. Bank Details
    a. Name of Account
    b. Account Number
    c. Branch of Bank
    4. Photocopy of Official Receipt that Bid Documents were purchased

    NOTE: All documents shall be current and updated and any missing document in the above-mentioned checklist is
    a ground for outright rejection of the bid. All pages of the documents shall be signed/initialed by the owner or by
    his authorized representative. Prospective bidders should submit one (1) original copy/certified true copy and two
    (2) copies of the eligibility and technical documents and financial documents.

    The abovementioned documents shall be arranged according to the checklist and properly labeled/tagged for easy
    checking.
