PREVENTION:- Advice to medical offcers to cross check the
medication chart.

WARD but the staff did not sign on medication chart.
RCA:- Staff was busy.
CA:- Provide training to staff proper signed the medication chart
after each medication with date and time.
PREVENTION:- Education & training given to all nursing staff

ROOM but same but new order charted in same coloum.
