Foreword

A Formulary System has been instituted at Majuro Hospital. A formulary


fosters the principles of good therapeutics, economics and efficiency of drug
utilization.

The Formulary is a continually revised list of pharmaceuticals that represents


the current clinical judgment of the medical, pharmacy and nursing staff.
Recommendations concerning the content of this formulary are made by the
Pharmacy and Therapeutics Committee.

The formulary also follows the principles of the World Health Organization
(WHO) Essential Drug List.

Essential drugs are those drugs that satisfy the health care needs of the
majority of the population; they should therefore be available at all
times in adequate amounts and in the appropriate dosage forms, and at a
price that individuals and the community can afford.

Prescribing Policy

Only licensed Doctors and Medex are authorized to write prescriptions. Prescriptions
should be written on approved prescription forms for Majuro Hospital.

The following guidelines should be used when writing a prescription:

• A full signature is required to make a prescription legal


• A prescription is required to contain the following details on it:
- name of patient
- hospital number of patient
- the date
- “location” of where the patient was prescribed the medications
e.g. OPD, ER, Public Health, etc.
- medication generic name
- strength of medication
- frequency of medication
- duration of medication (if applicable)
• use approved generic names for medications

These guidelines help ensure the safety of medication delivered to the patient.
